Mid-Missouri Limestone Inc · Struck by flying object

Employee was using sledge hammer to loosen hammer shoe in crusher. When struck, the shoe broke and metal shrapnel hit employee in hand and arm. Employee had stitches in right hand and left forearm.

Maintained awkward position while welding, resulted in strained knee / sore knee. Knee swelled, developed fluid on knee. Employee reported 5 days later and was treated through a prescription and light duty.
