Employee was walking around plant following up on facility inspection items. As EE was walking under the screen, the employee slipped on wet concrete causing EE to lose balance hitting head on the ground. EE sustained laceration to scalp requiring 7 stitches and was released to work without restrictions.

EE had completed a belt splice on the surge tunnel belt, and was applying a strap to the old belt to remove it from the conveyor. EE was descending from the work area, left pant leg caught a piece of rebar, causing EE to lose balance and stumble. Subsequently, the rebar caused a puncture to the employee's left calf. EE was given a prescription and placed on restrictions.

The injured EE was walking backwards and tripped over a return roller laying on the ground. EE felt discomfort in right shoulder requesting medical attention on 1/18/21.
