Pete Lien & Sons Inc · Struck against a moving object

7/12/2013 EE was driving truck on site, he drove over a bumpy portion of the mine road causing his elbow to strike the arm rest. EE reported injury on 7/15/2013 EE saw phys. on 7/18/2013-CT scan ordered shows bone chip on elbow. Follow-up reading sched. 8/6/2013 to determine if work related or an existing injury. Item being treated as reportable pending verification by phys.

Pete Lien & Sons Inc · Caught in, under or between a moving and a stationary object

Employee was changing a screen on a screen deck, he was pulling on an angle-iron rail and when the item was dislodged he pinched his thumb on his right hand between the rail and the screen motor. EE suffered a broken bone in his thumb and a contusion to the thumb nail.

While lifting the motor guard on HP400 cone with a crane, the picking bolt pulled out of the threads and the guard came down from the crane hook. As the guard came down, the corner of the guard hit ee in the face on the left cheek. EE was taken to the hospital where he received five stitches.

EE had cranked up the lights to turn them on. When he went to start them, the fiber clutch failed. The lights started to come down with the crank handle spinning. Ron tried to stop the handle and was hit in the left hand, on the ring finger.

Had crusher operator shut down Jaw to put a chain around big rock to move rock. Waited for jaw to stop and put chain around. Reached in between jaw dye and rock. The dye rolled crushing finger.

Big rock in Jaw Crusher. Shut down Jaw Crusher. Jaw had come to a complete stop. Reached around rock between Jaw Dye and rock. Jaw Dye moved inward about 2 inches, pinning my right arm in between rock and Jaw dye, causing swelling and bruising of the arm.

I was lifting a 60 lb. roller for a converger to get it out of way. I must of been twisting as lifting. Back popped and I fell to the ground. Could not get up for a couple minutes, but when I did get up, the pain was bad and I could not walk very well.

While on catwalk, I reached down through handrail to loosen bolts on flashing rail. My left leg was braced on the side of the conveyor. When I started to loosen the nuts, my leg slipped off and my left knee twisted as it hit the side of the railing.
