While drilling the drill steel came out of the rack and fell into windshield. EE tried to catch the steel with their left arm fracturing left radius at the elbow.

01/11/21 at 0730 miners foot slipped off bottom step of CAT 769 truck while entering and EE pulled left shoulder. Was taken to DR as precaution and released to duty with no restrictions pending MRI. MRI results were read 02/03/21 and EE received a cortisone shot and PT order which turned this in to a RECORDABLE.

Employee was cleaning out a plugged chute. When the material shifted, a 6 to 8 inch rock fell from the chute above, hitting his finger. He received a laceration which required stitches and the finger was broken.
Employee was using a prybar to dislodge a rock that was stuck in a chute. The rocks shifted and forced the prybar against the chute. His finger was mashed between the chute and the prybar. They removed his fingernail and the tip of his finger was fractured.

Employee was found unresponsive in the drill. Emegency Medical Services said he died from a heart attack.

Employee reported injury after using a 36" , 3/4 drive ratchet. The ratchet broke cleanly off. He was in the process of installing a counterweight on the cone crusher. It is unknown if this caused the injury, but it was the first time the employee reported it to the company. This injury requires surgery.

EE WAS TIGHTENING IMPACTOR BOLTS WITH A RATCHET. THE RATCHET SLIPPED A TOOTH AND HAD NO TENSION WHEN EE PULLED ON RATCHET HE HIT HIS ELBOW ON SIDE OF IMPACTOR FRACTURING HIS ELBOW.
EMPLOYEE CLAIMS HIS KNEE POPPED OUT A YEAR AGO BUT HE GOT IT BACK IN PLACE HIMSELF. HE SAYS IT HAPPENED AT WORK BUT HE DID NOT REPORT IT. SINCE THAT TIME IT HAS GONE OUT SEVERAL TIMES. HE H AD SURGERY ON HIS KNEE 8-22. HE WORKED AT HIS REGULAR JOB UNTIL THE DAY OF SURGERY.
EMPLOYEE WAS TURNING PULLEYS ON IMPACTOR TO PUT ON A NEW BELT. HIS GLOVE GOT CAUGHT BY THE BELT AND IT PULLED HIS HAND INTO PULLEY WHEN IT TURNED.

LIFTING CONE CRUSHER BASE WHEN A HOOK ON THE SLINGS CAME LOOSE AND HIT THE BACK OF HEAD.

A ROCK CAME OVER THE BUCKET ON LOADER AND HIT THE WINDSHIELD OF LOADER. WINDSHIELD EXPLODED SPRAYING GLASS ON THE EE.

EMPLPOYEE ALLEDGES HE WAS CRAWLING OUT OF EUCLID AFTER SHOOTING SHOT. HE SWUNG DOWN FRON EUCLID TO GROUND WITHOUT PROPER HOLD. TWISTED LEFT SIDE OF RIBS.
THE BELT HAD A TEAR TOO WIDE TO BE FASTENED BACK TOGETHER EMPLOYEE WAS CUTTING AROUND THE T EAR WITH A UTILITY KNIFE IN ORDER TO PLACE A PATCH IN THE HOLE OF BELT
EMPLOYEE ALLEDGES HE WAS SHOVELING ROCK OUT FROM UNDER IMPACT I AND TWISTED BACK.
EMPLOYEE ALLEDGES HE WAS CUTTING FEEDER CHAIN APART SO HE COULD REPLACE IT WITH NEW. HE WAS CUTTING OVERHEAD AND HOT SLAG FELL ON ARM. HE DID NOT BELIEVE THE BURN TO BE SERIOUS BUT SATURDAY MA Y 14,1994 HE NOTICED RED STREAKS GOING UP HIS ARM.
EMPLOYEE ALLEDGES HE WAS FEEDING BELT BACK ON IMPACT II PULLEY. HE GOT HIS RIGHT HAND BETWEEN BELT AND PULLEY.
THEY WERE CHANGING SCREEN ON PLANT. LIFTING SCREEN FROM GROUND TO PUT ON TOP DECK OF SCREENING PLANT.
EE WAS WORKING ON EQUIPMENT IN QUARRY. THE WIND BLEW A FOREIGN BODY IN HIS EYE.

EMPLOYEE WAS TAKING PIN OUT OF TOOTH ON 980 LOADER. HE WAS HAMMERING ON II WHEN A PIECE OF METAL FLAKED OFF & WENT INTO LEG ABOUT 2". THE METAL WAS CUT OUT & E STITCHES PUT IN.

EMPLOYEE WAS UNLOADING DRIL AND BUMPED HIS KNEE AGAINST A PIECE OF CHANNEL IRON EXTENDING FROM THE TRUCK.
