EMPLOYEE GATHERING UP EMPTY SODIUM HYDROXIDE PELLET BAGS TO BE THROWN AWAY. A TINY PIECE OF NaOH BOUNCED UNDER SAFETY SHIELD & BURNED RIGHT EYE. EMPLOYEE FOGGED GLASS SHIELD, RAISED HEAD BACK TO LOOK UNDER GLASS & THE LITTLE PIECE BOUNCED & LANDED IN EYE.

EE WAS DRAGGING A FUEL HOSE AND ATTACHING IT TO THE DISCHARGE FITTING ON A DC-6 AIRPLANE DELIVERING FUEL TO THE MINE. HE HAD UNLOADED SEVERAL PLANE LOADS OF FUEL DURING THE DAY. AT THE END OF THE DAY HIS LOWER BACK HURT.
