EMPLOYEE GATHERING UP EMPTY SODIUM HYDROXIDE PELLET BAGS TO BE THROWN AWAY. A TINY PIECE OF NaOH BOUNCED UNDER SAFETY SHIELD & BURNED RIGHT EYE. EMPLOYEE FOGGED GLASS SHIELD, RAISED HEAD BACK TO LOOK UNDER GLASS & THE LITTLE PIECE BOUNCED & LANDED IN EYE.
EE WAS DRAGGING A FUEL HOSE AND ATTACHING IT TO THE DISCHARGE FITTING ON A DC-6 AIRPLANE DELIVERING FUEL TO THE MINE. HE HAD UNLOADED SEVERAL PLANE LOADS OF FUEL DURING THE DAY. AT THE END OF THE DAY HIS LOWER BACK HURT.

EMPLOYEE STATED HE HAD SUFFERED VISION IN LEFT EYE CAUSED BY A WELDING FLASH OR DIRT FALLING INTO HIS EYE. EE WAS WELDING WEAR PLATE ON TO A DOZER FRAME AND ALSO BOLTING ON CUTTING EDGES.

EE REPORTS(AFTER THE FACT) THAT HIS RT HAND WAS POKED BY AN IRON RAIL. LEFT CAMP ON VACATION THAT AFTERNOON. HAND GOT INFECTED AND REQUIRED LANCING ON 2-26-01. EE DID NOT RECOGNIZE ANYTHING SE RIOUS ENOUGH TO REPORT.
DRIVING 4 WHEELER, HIT PIPE IN GROUND & LOST CONTROL. WENT OFF 4 WHEELER & HIT CREEK EMBANKMENT.
