While loading a face on level 3, NW2, the man basket of the powder truck raised up into the roof of the mine causing a fatal incident. The accident is still under investigation by the operator and the Mine Safety and Health Administration.

Cutting metal with a hand grinder, grinder jumped forward running across the employee's thumb, cutting the thumb. Employee was not wearing gloves, did not SLAM the task, and wrong tool for the job. The grinder cut the tendon on the thumb, the tendon was repaired and released back to work with restrictions.

Employee was placing a new sheet of metal in the bottom of a loader bucket. He welded two pieces of flat bar on each end to hold the sheet of metal in place and keep it from falling through. When he went to unhook the shackle from the metal he placed his left hand on the sheet. It flipped out of place and pinched his arm between the metal and the bucket.

Alledgedly hurt his back while loading 10/4 power cable into a p/u truck on 10/21,regularly off 10/22-24,worked 10/25,reported accident 10/26,worked 10/27,10/28 went to workers comp Dr.and was wrote out of work.First day missed 11/1/2010.Witness did not see him get hurt or show signs or was not told that he hurt himself. 11/1 mine idled workers transfered to another location.

Removing chunk from rear end of a 35 ton haul truck, when last bolt was removed the chunk fell striking employee in the head. He was taken to the hospital by company vehicle and received 4 stitches in right side of head. Failure to make secure and inattention to job hazard were the causes.

Miner Reported his elbow hurting on 5/16 from loading ANFO. He did not go to the Dr. till 5/25 and was diognosed with tendonitis, his arm is in a sling and he is on light duty for 2 weeks. This is a possible repetative motion injury.

EMPLOYEE WAS OPERATING A HAUL TRUCK AFTER DUMPING HIS LOAD HE FORGOT TO LOWER THE BED. HE THEN ATTEMPTED TO ENTER THE MINE WITH TRUCK BED UP. TRUCK CAME TO ABRUPT STOP CAUSING THE CAB TO OVE RTURN.
