EE WAS ROCK BOL7ING WHEN APPROX ONE TON OF BROKEN MATERIAL FROM THE ROOF FELL ON HIM KNOCKING DOWN INJURING HIS LEFT SHOULDER. EE DID NOT SEE DR AT THE TIME OF INJURY HE DID NOT THINK HE WAS INJURED BADLY 9NOUGH AT THE TIME BUT SHOULDER CONTINUED TO BOTHER HIM UNTIL HE SAW DR ON 1-13-87. HIS LOST TIME BEGAN ON 7-23-87 WHICH WAS HIS LAST DAY OF WORK-SURGERY ON 7-27-87.
