The employee was testing a skid loader that had finished being repaired by the on-site mechanic. The employee did not take his precautionary steps because he said he was just going to "test the machine." He got in and started up he did not have his forks down and the skid loader tipped over and the machine and fell over to its side with the worker still inside.
