Employee reported that employee was cleaning under C-1 belt on the MCC side and when employee went to get up, went blank. Employee noted that employee did not know if employee raised up and hit head on the structure or if employee stood up and slipped and fell and hit head. Employee stated that employee was wearing hard hat.

Employee reported EE was replacing sheave wheel on the 6X10 Diester screen when the wheel fell off the shaft and landed on right middle and ring fingers.

Employee reported that he strained his back while performing Jaw Crusher maintenance.

Employee reported that while cutting metal rail with cutting torch, piece of slag fell into his boot burning the front of his ankle.

Employee was one of two individuals welding at opposite points in primary crusher feed hopper. Employee was treated for welder's burns that night after suffering eye pain.

Employee was loading section of steel railing into the back of service truck when his middle finger of his right gloved hand got caught between the metal and truck bumper.

EE had symptoms of a stroke ee was transported to hospital and MSHA was notified. The ER physician determined that it was not a stroke but a reaction to a change of medicine. He was told to contact his family doctor in regard to the re-action. Medical Claim Only.

Employee was inspecting Pin Bushing when a metal bur cut his right index finger.

While opening the hood on the Mack Stock Truck he lost his footing and fell backwards twisting his ankle, falling to the ground.

Was working on dust collector using a pry bar when it slipped causing his right thumb to be struck with the bar.

EE was shoveling walkway around the cone crusher when 1/2" water pipe fell, striking thumb of left hand.

EMPLOYEE WAS PERFORMING PRE-SHIFT INSPECTION ON WATER TRUCK WHEN HE STEPPED OFF WATER TRUCK AND SPRAINED HIS RIGHT ANKLE.

EMPLOYEE WAS USING A 2" X 6" OAK BOARD AS AN IMPACT TOOL WHEN THE SPLINTER FROM THE WOOD WAS IMPACTED INTO THE PALM OF THIS LEFT HAND.

EE WAS SHOVELING - DOING CLEAN UP WORK UNDER A CONVEYOR BELT, WHEN HE PULLED A BACK MUSCLE.

Ee was putting a belt on Crusher Sheave when the pry bar he was using to hold the Sheave back came loose. His right hand became caught under the belt injuring his four fingers on his right hand.

EMPLOYEE WAS ATTEMPTING TO MOUNT AN OFF ROAD TRUCK WHEN HIS FOOT SLIPPED ON CHAIN STEP CAUSING HIM TO FALL BACKWARD SPRAINING HIS RIGHT ANKLE.

WHILE DRILLING A HOLE IN METAL PLATE USING A 1/2" ELECTRIC DRILL THE BIT GRABBED, JERKED DRILL MOTOR OUT OF GRASP, MOTOR SPUN AROUND STRUCK RIGHT HAND, BROKE BONE IN RIGHT HAND.

WHILE LOOSENING A TENSION SPRING BOLT ON THE 44 X 48 JAW CRUSHER, THE NUT BROKE LOOSE AND THE 12 INCH WRENCH SLIPPED OFF AND STRUCK EMPLOYEE IN MOUTH.

WHILE PERFORMING DRILLING DUTIES A SECTION OF DRILL STEEL FELL FROM UNSECURED ROCK AND STRUCK EMPLOYEE ON RIGHT HAND.

WHILE PRESSURE WASHING TRUCK EE TRIPPED OVER HOSE & FELL REACHED OUT TO BREAK FALL WITH LEFT HAND & GRABBED OR TOUCHED HOT MUFFLER ON PRESSURE WASHER

WHILE CLEANING UP AROUND THE CONE CRUSHER, A 4" ROCK BOUNCED OFF THE SCREEN AND STRUCK EE ON LOWER BACK. EE WAS SENT TO DR., EXAMINED AND RELEASED FOR LIGHT DUTY FOR 3 DAYS. EE WAS REPRIMANDED FOR WORKING IN A RESTRICTED ARE WHILE PLANT IN OPERATION.

EMPLOYEE ATTEMPTING TO CLEAR ROCK FROM BETWEEN BELT AND IDLER ON CONVEYOR #13. REACHED IN BEHINDSTEEL MESH GUARD, GLOVED RIGHT HAND CAUGHT BETWEEN IDLER AND BELT. PULLED HAND AND FOREARM INTO PINCH POINT BEFORE RELEASED. BELT LINE WAS RUNNING. BRUISED HAND AND FOREARM. DR. EXAMINED AND RELEASED EMPLOYEE TO RETURN TO WORK 11/20.
