THE EE HAD BEEN WELDING OFF AND ON ALL DAY DURING HIS WORK SHIFT AND DURING THE NIGHT, AROUND 1100 PM HIS EYES STARTED BURNING. HE WAS WEARING THE WELDING SHIELD. HE WENT TO DR TO GET SOME MED ICINE FOR THE UBRNING SENSATION.

EMPLOYEE WAS STANDING ON A STEEL I-BEAM APPROX 5' FEET OFF THE GROUND WHEN HE TURNED TO STEP DOWN HE LOST HIS FOOTING AND HIT HIS HEAD. HE SUSTAINED A SMALL CUT ABOVE THE BROW AND SEEMED DISOR IENTED. EMPLOYEE WAS TAKEN TO THE NEAREST EMERGENCY ROOM WHERE THE DR., UPON EXAMINATION, DETERMINED HE HAD A CONCUSSION AND ASKED THAT HE COME BACK IN 48 HOURS FOR RE-EXAMINATION.

EE LOST HIS FOOTING ENTERING THE LUBE TRAILER AND FELL. H STRUCK THE DOT BUMPER AND CUT HIS RIGHT SHIN. THE CUT WAS CLOSED AT COLUMBIA MEDICAL IN WHITWELL TN. IT REQUIRED 7 STITCHES. EE WA S RELEASED THE SAME AFTERNOON.
