EE WAS HOLDING WOODEN STAKE WITH LEFT HAND AND ANOTHER EE WAS HAMMERING STAKE INTO GROUND FOR A SILT FENCE. THE STAKE BROKE INTO AND EE SUSTAINED A PUNCTURE WOULD ON LEFT HAND REQUIRING 2 SUTU RES.
THE EE HAD BEEN WELDING OFF AND ON ALL DAY DURING HIS WORK SHIFT AND DURING THE NIGHT, AROUND 1100 PM HIS EYES STARTED BURNING. HE WAS WEARING THE WELDING SHIELD. HE WENT TO DR TO GET SOME MED ICINE FOR THE UBRNING SENSATION.

WHILE TIGHTENING A HYDRAULIC LIJNE THE WRENCH EE WAS USING SLIPPED CAUSING HIM TO HIT HIS HAND AND WRIST. EE WAS DRIVING TO SEE DR AND RELEASED TO FULL DUTY ON 7-1-98.

EE WAS PATCHING A TIRE WHEN THE PLUG BLEW OUT AND GLUE WAS SPRAYED IN HIS EYE. EE S EYE WAS CLEANED WITH EYE WASH AND WAS TAKEN TO A PHYSICIAN. EE WAS RELEASED TO FULL DUTY WITH NO LOST TIME.
EMPLOYEE WAS STANDING ON A STEEL I-BEAM APPROX 5' FEET OFF THE GROUND WHEN HE TURNED TO STEP DOWN HE LOST HIS FOOTING AND HIT HIS HEAD. HE SUSTAINED A SMALL CUT ABOVE THE BROW AND SEEMED DISOR IENTED. EMPLOYEE WAS TAKEN TO THE NEAREST EMERGENCY ROOM WHERE THE DR., UPON EXAMINATION, DETERMINED HE HAD A CONCUSSION AND ASKED THAT HE COME BACK IN 48 HOURS FOR RE-EXAMINATION.

EE LOST HIS FOOTING ENTERING THE LUBE TRAILER AND FELL. H STRUCK THE DOT BUMPER AND CUT HIS RIGHT SHIN. THE CUT WAS CLOSED AT COLUMBIA MEDICAL IN WHITWELL TN. IT REQUIRED 7 STITCHES. EE WA S RELEASED THE SAME AFTERNOON.
