Cut on right knee when struck sharp edge under dash on maintenance truck.

Reporting out of an abundance of caution. Employee reports a sore shoulder Monday morning. Reports it started hurting over the weekend, away from work. Reports no incident at work but feels it could have resulted from work. ILC questions causality.

While doing maintenance with a pry bar and chain fall, the chain fall hook broke resulting in the pry bar slipping, striking the employee's left arm causing pain and discomfort.

Employee claims discomfort in arms & shoulders while shoveling/sweeping. Employee was new hire and claims injury 1st hour of 1st day doing physical work. Pre-existing conditions are confirmed by doctor. WC fraud suspected and question if this is even MSHA reportable.

Miner bypassed two hydraulic safety devices to allow adjustment of bucket from outside of machine. Miner engaged hydraulic pedals by hand outside of machine, and under bucket. This allowed the bucket to apply force to miner's left side. Miner walked to job site office and called supervisor to go to the hospital.

Employee was attempting to free a overloaded conveyor on the No. 16 belt. During the process of removing the build-up of material the tail pulley and the belt rolled backward slightly pinning the employees right arm. Another employee assisted in freeing his arm, which was pinned at his elbow between the conveyor frame and a pry bar he was using.
