Miner was outside of breakroom with coffee pot. Miner noticed truck in plant and hurriedly turned to go back into breakroom. When doing so, Miner tripped stepping up into breakroom and fell to the floor with coffee pot in hand. Coffee pot shattered. Miner's right hand was cut including a piece of glass enbedded in palm of right hand.

Miners were making repairs to water pump on dredge. Miner went to get crescent wrench from motorboat. When stepping from dredge to motor boat, miner slipped and fell toward welding machine. Miner' s right leg contacted the lead holder causing a laceration. Miner received 9 sutures. Contributing factors: Welding machine in motorboat.

Miner was a bystander where welding on a piece of 30" pipe was being performed. Miner was exposed to welding flashes and injured eyes. Miner claims to have been turning as welding took place.

EE'S LEFT SHIN SCRAPED THE EDGE OF THE BARGE. 1ST AID WAS ADMINISTERED & EE RETURNED TO NORMAL DUTIES. ON 6/4/04 EE & MANAGEMENT FELT INFECTION WAS SETTING IN. ON 6/8 EE WENT TO DR & RX WAS GIVEN. RETURNED TO REGULAR DUTY. 6/11/04 SAW SPECIALIST.
