EE WAS ASKED TO CHECK A PTO LEVER ON A DUMP TRUCK HE BUMPED THE LEVER WHICH CAUSED THE PTO GEAR TO GRIND AGAINST THE TRANSMISSION GEARS. THIS SPUN THE PTO SHAFT HIS COVERALLS GOT CAUGHT BY TH E SHAFT WHICH BROKE HIS ARM. LEFT UPPER EXTREMITY HAD RECEIVED AN AVULSION & FRACTURE TO THE FOREARM EQUIPEMENT SHOULD NOT BE WORKED ON WITH ENGINE RUNNING
