A former employee alleges he tore his left rotator cuff at work. He mentioned one day in passing that his shoulder was "sore", but NO claim was summitted. He was dismissed in Sept. for multiple incidents; it is only now that we are hearing that an injury allegedly occurred.

While adjusting comealong EE over adjusted and had thumb in the wrong place. His left thumb was caught between comealong and angle iron.

TWO EMPLOYEES WERE REMOVING A PADDLE WHEEL FROM THE END OF A CONVEYOR BELT STACKER. THEY WERE USING A MANLIFT TO GAIN ACCESS TO PADDLE WHEEL. AFTER UNBELTING THE PADDLE WHEEL INJURED EMPLOYEE GRABBED SHAFT WITH HIS LEFT HAND & PULLED IT OUT OF BARRING MOVEMENT. SHAFT DROPPED 3" & SMASHED THUMBNAIL ON LEFT HAND BETWEEN SHAFT & ANGLE IRON FRAME. THUMBNAIL WAS REMOVED AT HOSPITAL.

EE WAS MEASURING A STEEL BAR THAT WAS LAYING ON A RAILROAD TIE. WHEN HE STEPPED BACKWARDS WITHOUT LOOKING, HE TRIPPED AND FELL. HE EXTENDED HIS LEFT HAND AND BROKE HIS WRIST.
