EE WAS USING A SLEDGEHAMMER TO POSITION A TOOTH ON THE BUCKET OF A HYLIFT, SO THAT THE KEEPER PIN COULD BE INSTALLED THAT HOLDS IT IN PLACE. WHILE HITTING THE TOOTH PIECE OF METAL, THAT APPEAR ED TO COME FROM THE TOOTH, FLEW OFF AND PENETRATED TO THE BONE, THE EMPLOYEE S LEFT ARM BETWEEN THE SHOULDER AND ELBOW.
EMPL WAS OPERATING A SCAPPER ON 2-1-89 EMPL EXPERIENCED PAIN IN HIS L ARM WHEN HE WENT HOME AND ADDITINAL PAIN SEVERAL DAYS LATER PINCHED NERVE IN HIS NECK
