4/1/2015 Employee had hip replacement surgery. 5/27/2015 Employee and his brother went into the mine to begin getting the mine open for the summer. Employee was on very light duty. Employee was in the cabin, sitting down eating lunch. Employee bent down to pick up a salt shaker off the floor and dislocated his hip. The mine was not operating at the time.
