Employee was cleaning out stone crusher before we started the plant. Employee was scraping compacted sand from the walls inside machine. They lost footing and their leg slipped between two metal pipes on the machine. Leg was bruised on either side where it was wedged against the metal. Sent them for x-ray to be safe, no damage found, leg was sore for a few days.

Employee was performing clean up on our screening operation. EE opened a heavy door and it slammed against EE's wrist. Employee returned to work that day, but decided to get x-ray that evening. Nothing broken, MRI ordered to confirm findings. Employee returned to work next day. MRI a few days later confirmed nothing broken. Employee remained at work.

Worker was splitting stone with hammer next to the fork lift truck. When EE swung the hammer EE accidentally hit the rubber tire of the forklift, the hammer bounced back and hit EE on the side of the head. There were not significant cuts or bruises but EE gave self a concussion.

Worker was loading material on trailer, tripped and fell to the ground. EE was ok, finished the day and the next day. After the weekend EE was still sore so we sent EE for X-rays which showed no break. EE took the rest of the week off, saw the doctor again 5 days later and returned to work.

Miner was sorting stone laid on a scrap pile in order to put it on a pallet. He stepped on a smooth stone and slipped. He fell on his side and cut his hip. The cut was not very big but looked deep so he got two stitches just above his hip.
Worker was splitting a stone with a rock hammer. He accidentally hit the tip of his index finger with the hammer. He was wearing gloves but still cut the tip of his finger. The finger was sore but did not require stitches.
Loader operator was pulling out of a parking space next to an area where employees were parking their cars. This area was tight and the operator scraped along the side of another employees minivan, damaging the sliding door and rear quarter panel. There was no one else in the area and there was no injury. The minivan is drivable and repairable.
