Kayenta Mine Coal

I was getting my toolbox on the bed of my truck after picking up the toolbox I turned to get down when I tripped on a jack which caused me to fall off my truck and I landed on the concrete surface head-first and I tried to break my fall with my left arm but I still hit my head. I received a cut on my head.

Employee was sorting 5 dragline cables. Employee had just started separating cable in the area where there was snow covered approx 6" - 8" of snow.

Employee was adjusting the two lengths of poly water pipe into the fusing machine when EE's left index and middle fingers got caught between the poly water pipes ends, which pinched the tips of the two fingers.

Smoke was noticed at 11:45 at N-8 hopper and found a small spontaneous combustion fire due to location the fire burnt for more than 30 minutes before it was extinguished.

Ford Truck collided with Powder Truck at N-14 Facilities intersection. Ford was headed east and Powder Truck was pulling out of a stop sign when they collided. Ford driver was transported to hospital treated and release with bruises. Powder truck driver was treated by paramedics and released.

Operator & Oiler were in process of moving power cable to maneuver Machine. Oiler hooked up 2" sling around power cable & using 1" sling hooking to fan tail hook. Before Oiler was ready Machine started to swing while Oiler's hand & fingers were still under sling on power cable side. Resulted in four fingers getting injured from the pressure of the sling when Machine swung.

Operator was dismounting a 785B End Dump, off a ladder when incident occurred. Operator missed the last rung, fell backwards to the ground. Put out left hand to break fall, fell on left hand. Fracturing left distal radius.

While employee was lifting 4 foot cribbing. EE felt a sharp pain in right shoulder. IE reported it to Supervisor and was taken to and seen at the Mine Clinic. IE was referred to another Clinic for further evaluation where Doctor determined it was a rotator cuff tear.

After employee finished replacing troughing idlers on beltline, ee was in the process of removing used idlers off the platform on the catwalk. To clear the walkways and got left hand caught in between handrail and idler frame, pinching the side of left thumb, receiving a laceration which required 7 stitches.

EE was assisting in installing bucket cylinder to linkage. A crane and sling were being used to pull cylinder forward to align. EE noticed a piece of plastic covering bore of cylinder and tried to remove it. This caused the sling to slide up, swinging the cylinder and EE's hand was caught in pinch point.

Trainee was moving cable at 2570. Trainee twisted as they were lifting cable and felt lower back pain. Reported to supervisor.

Walking cam on 2570 dragline was being installed and was being set into place by 992 cat loader. Cam seized and was rammed by the loader and jarred the operator, causing whip lash to the operator.

IE severed the left middle finger tip while installing a push arm assembly on a D-11 dozer.

A non miner suffered a heart attack at the public coal load out. While waiting for Navajo EMS to arrive, CPR was started. Once Navajo EMS arrived, the victim was transported by their ambulance to the mine helipad. Efforts continued to treat the victim with no response. Efforts were terminated by the flight nurse per hospital protocol.

Employee was exposed to unburned diesel fuel vapors.

The IE was lowering a tow pin weighing 25 lb. on the bumper of a water truck. As the IE was trying to lower the pin, it got hung up and wouldn't move. As the IE applied more force, the 25 lb. pin fell 3" to 4" inches and struck the IE's hand, causing a fracture to the left thumb.

As a scraper operator was running in the spoil cut area, was looking back at the apron and bowl and hit a large partially covered boulder. As operator made contact with boulder, the scraper bounced hard and the seat bottomed out causing operator to strain lower back.

The EE was in the process of removing a hydraulic filter from drill 812 when EE injured a middle finger. EE first removed the bracket that secures the filter housing assembly. EE then held the filter housing assembly with left hand and began to remove four bolts located on top. When EE removed the last bolt, the filter housing dropped and crushed the left middle finger.

When troubling shooting a limit switch, on a load out chute and is 25 feet above ground level. The electrician was utilizing mobile scaffolding to access the limit switch. The electrician adjusted the switch and raise the chute. When the switch was activated, the chute began to rise and struck the scaffolding. EE fell, struck the scaffolding and fell on concrete floor.

As a dozer operator was dismounting dozer 327 at the end of EE's shift, EE failed to use the workable power ladder installed and used the iron grab irons to dismount. When EE got to the left push arm, EE jumped to the hard ground where there were small baseball size rocks. Upon landing EE twisted EE's left ankle and twisted EE's left knee falling on EE's right shoulder.

The dragline operator and oiler were in the process of inserting a shaft roller on top of the dragline house. The operator lost hold of the shaft roller and it slipped from hands, left middle finger was pinched between the shaft roller and housing causing a laceration. Over the weekend the wound became infected.

As backhoe 450 was loading Haul Truck 409, large chunky material was dumped into the bed of the truck. The shock load of the rocks being loaded caused the truck driver to suffer back and neck pain. The IE did not report to their next scheduled shift.

A serviceman injured his right shoulder while cutting a padlock off a truck's tool bin with bolt cutters. Afterwards he swung his arm to test and relieve the pain when his shoulder popped. This caused dizziness and numbness to his arm, forcing him to kneel. After regaining strength, he rose to report it. He was seen locally and in Kayenta where a strain was diagnosed.

Employee was working onsite on the drill pad and turned to the right stepped on the edge of a rock and twisted knee.

EE in an attempt to remove broken bolts from a dozer track pad, lacerated the heel of his gloved left hand. EE had positioned his left hand along the outer edge of the pad, then manhandled the freed pad towards him, when it slid into the adjacent pad pinching his hand. The EE was taken to the Kayenta Health Clinic where the laceration was sutured.

Employee strained his back dismounting a dozer.

The 8200 dragline oiler dismounted the machine carrying a bucket of oil when he stepped on an unhinged extension ladder that was laying at the base of the catwalk. He twisted his right ankle. An x-ray revealed a right ankle fracture.

331 dozer pushing coal into N-8 hopper. Dozer went over concrete edge of hopper and slid forward into hopper. Angle of dozer cut fuel to engine. Dozer operator stayed in cab awaiting fuel to track out dozer. Operator exited cab shortly after shift change to allow other operator to track out dozer once fuel arrived.

Employee was resetting calibration limits on the 8750-1 dragline drum for a re-socket. When done, he was walking back to his work truck and twisted his left ankle on the ground transition from compact to soft and rock material beneath him. He refused Mine clinic treatment but went to the hospital later that night, where they diagnosed a sprained ankle.

Injured tripped over a 4" x 4" piece of cribbing in the motor shop while moving dragline house fans with the overhead crane resulting in a fracture to the right shoulder.

During re-socketing of 8750-1 dragline hoist rope, Employee was positioning the wedge into socket when the hoist rope loop being pulled by a dozer snapped and hit his right thumb. Employee reported at end of shift. He visited Mine clinic and had minimal bruising/swelling. The next day, a hospital doctor diagnosed a bruised thumb ligaments and tissues. He was excused for 3 days.

Employee slipped on gravel while loading cable on to a reel truck at the N-9 Cable Yard twisting his ankle and resulting in a left knee strain.

Injured employee, assisting a vendor in installing an A/C fan motor on the roof of the PCR room of a 2570 dragline, stepped on a 2" pad eye and twisted his right foot inward. He suffered a fracture to his 5th metarsal upon contact and was taken to the Kayenta, AZ clinic for treatment. Later,was referred to a Flagstaff, AZ orthopedist for further evaluation and treatment.

The serviceman started the light plant to service it when he noticed a cable lying underneath the plant. He knelt to move the cable when he received a shock from the damaged energized cable. He was able to free his hand but received burns to fingers.

The operator took a step on the push arm-step to mount the dozer when he heard a sound, a (pop) in left foot. Operator has experienced a chronic leg/foot ailments related to health and received treatments and compensations for days away from work.

The injured employee sustained laceration to his left pinky when a metal catwalk fell on his left hand after losing his grasp. He was initially seen at the mine clinic and later taken to Kayenta for further evaluation. The finger was not fractured but sutures were needed to close the cut.

Employee while assisting two other individuals in replacing a troughing idler, suffering injuries to his left wrist/hand. The employee's hand was pinned between the idler and the conveyor belt frame. The crew had managed to nearly upright the idler when a wrench used in process slipped off the idler causing it to fall on the injured limb, possible navicular fx.

Mechanic was removing and replacing bit and sub on drill steel when it shifted. Thumb got caught between steel and nylon sling. Mechanic sustained broken and laceration to his thumb.

Employee was removing crusher liners when he pinch his finger.

Employee was installing a conveyor belt clamp, and was holding the clamp when it got hang up. The clamp was being moved by a forklift, fork got hung up released and jarred him. This release jarred him and hurt his shoulder.

Employee was cleaning trash in scraper parking lot. Lifting a 55 gallon barrel full of trash. Was taken to hospital for further evaluation, and was diagonis with lower back injury. Was given one week off and to get more treatment.

Maintenace was being conducted on a dragline when incident occurred. Hoist rope was being changed out. Winch line was attached to hoist rope to remove it. Winch line got hung up on lug, released and struck worker above left ankle causing a fracture.

Public trash dumpster on mine site. Someone put hot ash in dumpster in a 55 gallon drum. Which cause the trash to ignite. Fire was put out with fire truck and dumpster was flooded with water. Fire was put out at 1300 hours. There were no injuries to any personnel.

Mechanic was observing another mechanic removing final drive of a dozer. Tool: bracket fell and mechanic was trying to get out of the way, trip and fell on his face. Causing a 1/2 inch laceration on his right forehead, which required three stitches. Mechanic was treated and released back to work.

The injured employee's left hand was caught in between the frame and the pivoting idler base in a scissor like fashion pinching the left ring finger between the 2 pieces of steel. The finger tip, about 1/4" was amputated. The IE was wearing gloves at the time of the incident.

Mechanic was in a sitting position when a co-worker walking by threw a piece of rock at him striking his right. Right hand sustained a simple fracture of a metacarpal of little finger.

Injured was trying to turn the swivel of a portable light plant into position and when it came loose, injured's right little finger struck the light plant mast stopper causing a cut to the finger tip.

Preparation laborer was cleaning with a water hose when his leg became wrapped by the hose. He took a step forward he lost his balance and fell backward striking the back of his skull against a concrete floor.

The Prep-laborer reached with his right hand to remove a steel choker while a splice house was lowered onto a lowboy trailer. The cross brace of the splice house and surface of the trailer pinched his right mid-finger. The finger sustained an avulsion requiring sutures to close the wound.

The loaded truck came out of the pit when it veered across the road and struck a road berm and jackknifed on top of berm.

A stranded haul truck right rear dual tires caught on fire. Tire friction with hard ground surface generated heat and fire and it was not extinguished within 30 minutes.

Injured dropped a 4"x32" bolt on his gloved right index finger while bolting the dragline hoist case together. Lacerated finger required 3 stitches at a local hospital and subsequently returned to work completing his shift.

The injured was maneuvering a belly pan to install it on a dozer when he slipped on a piece of ice. He fell backwards landing on his left shoulder and injuring his lower back.

The injured right index finger was pinched between a pipe wrench and the hoist brake shoe of a dragline. An impact wrench was activated from the opposite side to rotate the adjustment bar of the brakes, when the injured' s hand was jerked free pinching his index finger.

Security officer backed into occupied parked vehicle. Testing revealed the officer was under the influence of alcohol, police contacted, did not respond to scene. No injuries reported until the following day. Damage only to personal vehicle. Extent of reported injuries unknown at this time.

Officer responded to a roll over vehicle accident in inclement weather. While standing at her vehicle, the wind blew the door which struck her in the left elbow.

The mechanic was removing the dust filter collector cone when it slipped from his grip and dropped on his left ring finger.

The mechanic was stringing a sling through dozer blade cutting edge bolt holes with the cutting edge in tilted position. A pry bar was used to hold the cutting edge in place, slipped causing the cutting edge to drop on mechanic's right hand/fingers.

Hand crushed between actuator rod and park brake actuator.

Officer was working a double shift on security patrol. Fell asleep at the wheel and went off the road. Conditions were clear and dry. Injuries included contusions to his left elbow and left knee. Vehicle damage was primarily to the front suspension, 2 airbags deployed, minimal body damage.

The mechanic was holding the sides of the planetary cover of wheel assembly and putting it in place onto the housing when his right mid-finger was pinched.

Lower back/twisted pelvis using a pin removal tool to remove the equalizer pin from the roller frame the tool followed the pin into the bore. The pin came out and equalizer bar misalignment caused the bar to shift against and side load the tool. The tool handle struck his hand and causing his body to twist putting a strain on lower back.

The dozer left the work area and traveled along the edge of the highwall and came to a stop. When the dozer went into reverse, the ground underneath the left track gave way causing the dozer to fall off the highwall. The dozer landed in upright position at the base of the highwall.

A dragline operator was observing a winch cable being pulled by a nylon rope when the nylon rope broke. The elastic force of the nylon rope struck him from the left side knocking him to the ground.

The scraper was traveling on a scraper road to pick up another load when it collided head-on with a service truck traveling on opposite direction. The scraper operator was jarred inside the cab and reported pain to left shoulder and chest area.

The dragline oiler was holding a pry bar as the dump rope wedge was being hammered into the socket when a lift chain holding the wedge came loose. The loose chain struck the oiler's skull when his hard hat fell off.

EE fell while walking on grade (level) towards the back of a piece of equipment and injured his right leg. (fracture)

Employee strained wrist while climbing off machine.

Serviceman fell backwards from his truck landing on his back striking his head against the ground.

A small spon-comb activity was discovered with minor intermitted flames on highwall @ J-19 pit - Ramp #44. The spon-comb activity was not a hazard to workers and no pit activity was in progress. MSHA inspector was @ the site when flames were extinguished and under control. This report is submitted under protest.

The loader operator was closing the cab-window door when his right little finger was pinched between the door and jamb of the door way.

The scrap metal yard was discovered prior to shift change with smoke and spots of small flames at the Preparation Plant scrap yard. A water truck was summoned to the scene extinguishing the fire within (50) minutes. The water truck had to travel from the pit to the Preparation Plant allowing time to proceed beyond the (30) minutes threshold limit.

The rubber tire dozer operator claims low back pain after his shift and informed he has a scheduled chiropractor appointment the following day.

The oiler tripped over a metal cover while walking inside the dragline, striking his right shin against the edge of the metal cover.

The welder was cutting a dragline dump rope. When it was cut loose, the released energy in the twisted rope whipped out hitting the left knee.

A pair of scrapers were excavating top soil using (push-pull) method. The rear scraper rode up onto the front scraper's stringer area, when the front scraper hit a soft a soft pocket of material; jarring the rear scraper operator. He was jarred again as he was trying to remove the bale from the front scraper's hook. He felt pain to his back.

Employee went to start service truck and let warm up, there was a patch of ice that was covered by snow, he walked on this area and slipped, twisting left knee.

Hydraulic pressure caused spool to shoot out and pin hand between spool and frame.

The supply-man was pulling on a cardboard box of parts on the truck flatbed when the box tore at the sides, causing him to fall backwards against the truck head-rack. He struck head against a channel iron bracket lacerating the base of his skull. The incident occurred when he was in the process of unloading hoisting parts onto the dragline.

The scraper started to proceed down an incline following the curvature of the road when the tail-end began to skid towards the outer edge of road. The scraper bowl contacted the ground and the push block also nosed into the road, causing the scraper to roll several times down an incline. Operator escaped through the rear cab window after unsecuring his seat belt.

The dozer operator was standing on the cab platform lifting a nylon sling, when he lost his balance and decided to jump clear of the tracks and landed feet first on the ground. He injured his left forearm when it hit the side blade arm when he jumped off his dozer.

The mechanic was closing a fuse panel door on a loader after he made the necessary repairs when it slipped out of his left hand. The panel built-in rotating fans of air condition struck his left thumb lacerating it.

The welder was walking on a catwalk when he sprained his left ankle causing him to fall through an opening at the end of the walkway. He fell onto the next level injuring his ankle when he landed on metal surface.

The welder was changing out oxygen bottles gauge in the back of his welding truck. When he lifted a protective bottle gauge cover-door to perform the task, the cover-door came back down striking his left hand.

The welder was loading junction boxes onto lowboy trailer using a loader to lift them. As he attempted to make another lift the load shifted sideways tilting the loader and turned it over on its side. Operator struck his left knee against the steering wheel injuring it.

The welder placed power cable on dragline tub cable-hook and stepped back, when his left foot slipped underneath tub that was coming back down during deadhead of machine. Left foot-toes were pinched between the ground surface and the tub.

The injured was dismounting a dragline when the portable ladder gave away from underneath him. He fell sideways onto the dragline shoe sustaining injuries to his face and bruising his right shoulder.

Crew was positioning plate...EE was welding with back to plate...plate slipped & fell puncturing EE behind lt ankle.

The technician was greasing on the sample system when his right hand was caught between tertiary blade cutter and a limit switch bracket; lacerating the palm of his right hand.

EE was using pipe beveling machine wearing work gloves...thumb of right glove got caught in machine pulling EE's thumb into machine...removing nail & left portion of thumb to bone.

Reparing a Drill Tech Drill, the employee descended a ladder and caught his left hand on railing, cutting his left finger, requiring stitches

EE was preparing to enter base/tub...got his clear safety glasses out & sat them next to manhole...he was wearing tinted glasses...exhaust sucker fan sucked his glasses into the fan...he turned off the fan & reached in to retrieve the glasses...evidently the fan had not stopped. broken fingers & multiple lacerations.

EE was picking up trash and filling into 4x4x2 cardboard box. When trash was full the injured lifted the box and set it on 2 feet berm to push it over. Apparently he stepped in loose dirt slope causing him to slip lose his footing backward & overextended rapidly. Afterward, EE felt pain in his calf muscle.

The welder was prying out a shovel cable that was caught between the crowd bull-gear and it's guard when it broke loose. The cable struck the welder's left eye, face and his left little finger. The impact from the cable also caused him to fall backwards onto the crowed track below.

The haul truck went off the side of the road traveling over a berm and onto a lower section of a ditch terrain. The operator was jarred around inside the operator's cab causing multiple injuries. Initial assessement of injuries reveals laceration to chin, lips, pain to right leg and to lower back.

The injured was trying to re-start stalled but backhoe on scraper haul road. He stated that he climbed out of cab to spray some starting fluid into air filter housing. The injured move quickly to climb back into cab but slipped on the steps and lost his footing that caused to extend his left leg unexpectedly. felt pain after the accident.

EE stepped on uneven floor surface when stepping thru access hole in revolving frame & turning at the same time. He lost his footing & dislocated his left knee. Nothing was broken...put back into place. EE returned to lite duty work on 1-9-06.

Injured was trying to remove a barrel-plug by hitting it with a piece of metal strap when he missed it. He struck his right thumb against the sharp edge of plug head; lacerating it.

THE INJURED WAS WALKING/OBSERVING ON THE NORTH SIDE OF A CRUSHER WHILE IT WAS BEING CLEANED WITH A HIGH PRESSURE CLEANER WHEN A PROJECTED PIECE OF ICE FLEW OFF FROM THE PRESSURIZED SPRAY HITTING HIS LEFT EYE. INJURED WAS WEARING EYE PROTECTION AT THE TIME INCIDENT MINIMIZING THE SEVERITY OF THE EYE INJURY.

The welder was setting a pear-link Metal on a table when it slipped off. It caught his left hand between the linke and load line of an overhead crane. He was operating the crane controls with his right hand and wearing protective gloves which minimzed the laceration on the dorsum side of his left hand.

EE was welding & grinding under base...wind blowing 30+ mph. Went to clinic for removal of foreign object from eye...antibiotic prescribed.

Sometime during his workshift on 9/28 EE got sweat & foreign objects in eyes. Wind blowing 30 + mph. When woke on 9/29 eyes were irritated. Went to clinic ...no objects found in eyes but antibiotic prescribed.

ALTERCATION BETWEEN TWO EMPLOYEES RESULTED IN ONE OF THEM RECEIVING A CUT ON HIS LOWER LIP FROM A PUNCH THAT WAS DELIVERED BY THE OTHER CO-WORKER.

The operator was dismounting his equipment when his right foot slipped off the step. He fell down twisting his right ankle onto an uneven ground surface, injuring it.

The cable of the belt wrap itself around an idler. Bent a support beam & forced a pillow block bearing out of roller shaft, frozen idler w/friction from the tolling belt created heat ignating the rubber coating of the roller,the belt communication line, stopping the belt and burt 2 section of the conveyor belt. fire extinguished 45 minutes later & water truck.

EE WAS WORKING ON A FENCE PROJECT. EE WAS PULLING THE BARB WIRE AND JERKED THE BARB WIRE WHEN IT HUNG UP ON A SHRUB. THE BARBED WIRE PENETRATED THROUGH THE GLOVE AND CUT EE RIGHT HAND INDEX FINGER. THE CUT WAS ABOUT 1/2" IN LENGTH ON SKIN

THE WELDER WAS STANDING TO THE SIDE APPROXIMATELY 20' AWAY FROM WHERE A HYDRAULIC HAMMER WAS HITTING ON A ROPE SOCKET WEDGE TO FREE IT, WHEN A PIECE OF METAL SLIVER FLEW OUT AND HIT HIS RIGHT CHEEK.

THE EMPLOYEE WAS UNROLLING A FIELD FENCE WHEN HE WAS INJURED. HE WAS PUSHING ON FIELD FENCE TO GO OVER A BIG SHRUB AND HE SUDDENLY SLIPPED. THE FIELD FENCE ROLLED BACK TOWARDS HIS FACE AND HIT HIM. THE INJURED SUSTAINED AN ABRASION, A CUT ON HIS INNER LIP AND A CHIPPED TOOTH.

THE WELDER WAS USING A WRENCH TO PUT A 2 1/4" NUT ON A BOLT WHEN HIS RIGHT LITTLE FINGER WAS PINCHED BETWEEN THE WRENCH AND NUT WHILE WORKING ON THE SEGMENT BASE BOLTS.

The oiler was kneeling on the propel shaft on the dragline, tightening a nut on the intermediate shaft bracket-bolt when the propel shaft rotated, causing him to lose balance and fell forward hitting the compartment wall with extended left arm. He dislocated left shoulder when he collided with the wall.

The serviceman (greaser) was helping a welder install a handrail on a dozer when he grabbed an unsecured rail causing him to lose his balance and fell backwards. He landed on concrete floor with both feet.

EE WAS HELPING REMOVE CENTER PIN BOLTS WITH A PNEUMATIC IMPACT WRENCH AND A SWIVEL EXTENSION WHEN THE SWIVEL & BALL SOCKET CAUGHT HIS RING FINGER AS HE ACTIVATED THE TRIGGER OF THE IMPACT WRENCH WITH THE OTHER HAND. THE ROTATING HAZARD OF THE SWIVEL EXTENSION AND BALL SOCKET INJURED HIS FINGER.

EE CAME OFF J-21 MAIN HAUL ROAD GOING AT APPROX. 45 MPH FROM K MINE FACILITIES, MADE A TURN GOING TO J-21 EXPLOSIVE YARD, APPROACHED THE EAST GATE GOING ABOUT 15 MPH & TRIED TO APPLY THE BRAKES. BRAKE PEDAL JUST FELL TO THE FLOOR, STEERING WHEEL LOCKED UP. FRONT OF VEHICLE WENT UNDER THE GATE - WINDSHIELD SHATTERED, GATE OPEN BROKE SIDE POST.

THE WELDER WAS PERFORMING HOUSEKEEPING DUTIES IN THE SHOP & AS HE WAS MANEUVERING & PLACING A CRUSHER SLIDE PLATE IN AN UPRIGHT POSITION, THE INNER ADJUSTABLE PLATE SLID DOWN CATCHING HIS LEFT RING FINGER.

THE WELDER WAS REMOVING AN EXPANDED METAL GUARD OFF A FLAT BELT WHEN IT HANG-UP WHEN HE TRIED TO REMOVE IT. HE PULLED ON IT & IT CAME LOOSE ON HIM CATCHING HIS LEFT RING FINGER BETWEEN THE GUARD & IT'S BRACE.

THE INJURY WAS PICKING UP TOOLS AFTER THE BACKHOE BUCKET REPAIR. HE PICKED UP CHIPPED HEAD SLEDGE HAMMER BY THE HEAD WITH SHARP EDGE THAT CUT HIS TIP OF LEFT LITTLE FINGER.

EE & COWORKER WERE CLOSING TEMPORARY FENCE GATE & SECURING IT W/SMOOTH WIRE ATTACHMENT OF WOODEN TREE BRANCH. EE WAS PULLING ON WOODEN TREE BRANCH TO SECURE IT TO WOODEN POST & HIS COWORKER WA S PUSHING & PULLING ON T-POST & FENCE GATE THAT ARE TIED TOGETHER. AFTER HIS COWORKER REMOVED HIS HAND FROM PULLING TO ASSIST EE IN SECURING TREE BRANCH ON WOODEN POST, THE GATE FENCE JERKED

THE TECHNICIAN WAS CLEANING AROUND THE PULLEY BY REMOVING FROZEN COAL WITH A PRY BAR WHEN HE APPARENTLY HIT A GUARD WITH HIS HAND CAUSING THE PRY BAR TO STRIKE UP & HIT HIS RIGHT CHEEK.

The welder was moving a pallet box full of quipment parts by hand when he felt backwards, after the box he was holding tore off during the pull. he strucked his head against an argon bottle rack directly behind him.

METAL STAIRS AND RAMP WERE MOVED 20' AWAY FROM OFFICE TRAILER SO MUD COULD BE CLEANED UNDERNEATH STAIRS, RAMP AND DRIVEWAY. WHEN EE WALKED UP THE STAIRS TO HOOK CHAIN BACK ONTO FRONT END LOAD AFTER THE DRIVEWAY WAS CLEAN, COWORKER MOTIONED FOR THE LOADER TO MOVE FORWAR AND WHEN HE DID, EE STEPPED BACKWARD AND FELL OFF THE 3' LANDING ONTO HIS BACK.

THE WELDER WAS HAMMERING ON A BUSHING INSTALLING IT INTO A CHAIN LINK WHEN HIS (R) INDEX FINGER WAS CAUGHT BETWEEN THE HAMMER HANDLE AND THE BUSHING.

THE OPERATOR WAS LEAVING THE PIT AFTER HE WAS LOADED WHEN HIS TRUCK HIT A SOFT SPOT IN THE PIT FLOOR (ROAD). HE WAS JARRED INSIDE THE CAB CAUSING LOW BACK PAIN. HE MOMENTARILY BLACKED OUT AND EXPERIENCED SHORTNESS OF BREATH.

THE MECHANIC WAS SWINGING A HAMMER TO KNOCK OUT A PIN WHEN IT GLANCED OFF EQUIPMENT FRAME AND STRUCK HIS LEFT WRIST. HE EXPERIENCED PAIN IN HIS WRIST FROM THE IMPACT AND ALSO FELT PAIN IN HIS LEFT SHOULDER.

THE PREP LABOR WAS PUTTING TOOLS BACK INTO THE TOOL BOX WHEN HE TRIPPED ON TOOL BOX FLOOR EDGE OR LIP OF BOX AS HE TOOK A STEP BACK. HE FELL STRIKING HIS HEAD AGAISNT THE CAROUSEL BRACKET.

TRIPPED WHILE STEPPING OVER STATIONARY EQUIPMENTTEMPORARILY STORRED AT STE 1 CRATES.

THE WELDER WAS INSTALLING TROLIGHING IDLER DURING BELTLINE REPAIR WHEN HIS RIGHT RING FINGER WAS PINCHED BETWEEN AN IDLER FRAME AND THE BELT MODALE.

THE SPOIL FIRE WAS REPORTED TO MINE MANAGEMENT IN J-21 PIT AREA @ RAMP 36 & RAMP 37. (NOTE: SUBMITTING THIS REPORT UNDER PROTEST)

THE OILER (EE) WAS CLEANING THE LOWER FRAME BY THE INTERMEDIATE GEAR OF THE DRAGLINE WHEN HE LIFTED A METAL PLATE BUT SLIPPED OUT OF HIS HAND AS HE MOVED IT. THE PLATE STRUCK LT. FOOT WHEN IT DROPPED TO THE FLOOR.

MECHANIC WAS DRAINING HOT OIL OUT OF DIFFENTIAL HOUSING WHEN THE DRAIN COVER BLEW OFF AFTER THE COVER BOLTS WERE LOOSEN AGAIN TO COMPLETE DRAIN.

THE MECHANIC WAS USING A HAMMER & A SOCKET AS A PUNCH TO REMOVE A SHAFT CARRIER BEARING OFF A LOWBOY TRAILER WHEN HE HIT HIS THUMB AGAINST THE BEARING HOUSING. THE HAMMER DEFLECTED OFF THE TRA ILER FRAME AS HE WAS TRYING TO HIT THE SOCKET-PUNCH.

THE MECHANIC WAS CHANGING OUT A DRILL BIT ON THEDRILL STEM WHEN THE DECK BUSHING ROTATED BY THE OPERATOR CAUSING THE DECK BUSHING TO DROP WHEN THE HOLDING SLING WAS PULLED/SLIPPED LOOSE. THE D ECK BUSHING PINCHED TWO RIGHT FINGERS AGAINST THE DRILL BIT.

THE WELDER WAS CUTTING ON THE DRAG CABLE WHEN IT CAME LOOSE FROM THE SOCKET STRIKING HIM IN THE LEFT LEG.

THE MECHANIC WAS CLIMBING DOWN OFF A DOZER WHEN HE LOST HIS FOOTING CAUSING HIM TO FALL BETWEEN THE MACHINE AND THE ROLLER FRAME. HE HIT THE RIGHT SIDE OF HIS RIBS AGAINST THE ROLLER FRAME AS HE FELL TO THE SHOP FLOOR.

STARTED UP LADDER ON SIDE OF TRUCK WITH 5 GALLON GAS CAN IN HAND. WHEN STEPPED UP TO SECOND STEP HE LOST HIS GRIP CAUSING HIM TO LOSE HIS BALANCE AND FALL. HE FELL ON UNEVEN GROUND TWISTING HI S KNEE.

THE MECHANIC WAS LOOSENING ALTERNATOR BOLTS WITHTWO WRENCHES WHEN ONE WRENCH SLIPPED. THE SECONDWRENCH USED AS LEVERAGE JAMMED INTO HIS PALM CAUSING A CUT.

INJURED WAS CLEANING OUT THE METAL TRAIL CABLE CROSS OVER WITH HIS HAND WHEN HE CUT HIS RIGHT HAND CONTACTING A SHARP EDGE.

THE INJURED WAS TRAVELING IN HIS PICKUP TRUCK BEHIND A WATER TRUCK AND AS HE TURNED LEFT INTO A PIT RAMP, A VEHICLE TRAVELING THE OPPOSITE DIRECTION SLID INTO HIS TRUCK ON THE LEFT SIDE.

EMPLOYEE PREPARING TO BLEND EMULSION UNHOOKED AN SOLUTION HOSE AND SPILLED A SMALL AMOUNT OF HOT SOLUTION ON LOWER LEG AND ANKLE; HE WENT TO FIRST AID STATION FOR ANTISEPTIC AND BANDAGE. ON 9 /6/00 HE WENT TO CLINIC BECAUSE OF INFECTION.

AT ABOUT 0840 ON 8-24-2000, THE INJURED WAS UNLOADING STRAW BALES IN OUR YARD AT THE OLD BUCKET SHED FROM STRAW SUPPLIERS TRUCK. INJURED EE FELL APPROX. 15' FROM TOP OF BALES STRACKED ON TRUCK , LANDING ON RIGHT SHOULDER AREA. MINE NURSE WAS NOTIFIED AND RESPONDED WITH AN AMBULANCE, TRANSFERRED TO CLINIC THEN ON TO CITY BY MEDICAL AIRPLANE.

EE WAS LEAVING HIS WORK AREA, END OF THE SHIFT, HE STEPPED THROUGH AN OPEN MANWAY THAT IS HINGED. THE MANWAY WAS LEFT OPEN BY SOMEONE ELSE AFTER HE WENT INTO HIS WORK AREA.

THE WELDER WAS WORKING ON HAUL TRUCK TRAILER SETTING METAL PLATE ON BACK OF TRAILER, WHEN HE MISJUDGED STEPLADDER HE WAS WORKING OFF AND FELL APPROX. 3' HITTING HIS HEAD ON CONCRETE FLOOR.

PERSONNEL WERE UNLOADING A SHIPMENT OF BLASTING ACCESSORIES THAT ARRIVED THAT MORNING. BOXES OF BOOSTERS (65 LBS) WERE BEING STORED AND STACKED. EE WAS REARRANGING AND STACKNG SOME BOOSTERS WH EN HE TWISTED AND HEARD A LITTLE POP IN HIS BACK SO HE WENT DOWN TO THE CLINIC.

MECHANIC WAS ROTATING A CARRIER ROLLER ON A DOZER WITH THE TRACK SUSPENDED WITH HOIST. THE CHAINSLIPPEDON TRACK CATCHING HIS RIGHT HAND BETWEEN ROLLER & TRACK

THE MECHANIC WAS TROUBLESHOOTING THE HEATING SYSTEM ON AN EQUIPMENT, WHEN THE ACCESS PANEL FELL OVER STRIKING HIS LEFT ELBOW.

INJURED WAS INSTALLING "O" RING IN A CYLINDER HOSE WHEN THE TORQUE FROM THE HOSE THREW HIS RIGHT HAND INTO THE FAN CAUSING LACERATION.

THE WELDER WAS HITTING THE BOTTOM END OF 1"X3'X4' RE-INFORCEMENT METAL PLATE ON THE SIDE OF A DRAGLINE BUCKET, WHEN IT CAME LOOSE AND HIT HIM ON HIS UPPER BACK.

THE WELDER DECIDED TO CRAWL ACROSS THE HOPPER-GRIZZLY DURING REPAIR. HE TRIPPED AND FELL FORWARD STRIKING HIS LEFT HAND AGAINST THE SIDE OF THE HOPPER-GRIZZLY.

THE WELDER WAS REMOVING THE BELT CLAMP AND AS HE LIFTED IT DROPPED ON HIS RIGHT MIDDLE FINGER.

INJURED WAS MOUNTING EQUIPMENT WHEN HER RIGHT FOOT SLIPPED OFF MOUNTING STEP CAUSING IT TO INJURED HE LEFT KNEE, WHEN SHE TWISTED HER LEFT LEG.

EMPLOYEE WAS WALKING ACROSS ON THE BACK SIDE OF THE MACHINE(DRAGLINE), WHEN HE STEPPED ON AN AIRHOSE AND TWISTED HIS LEFT ANKLE.

THE INJ CLAIMED THAT WHEN CLOSING THE DOOR ON 974 4X4 3/4 TON TRUCK HIS FINGER GOT CAUGHT IN BETWEEN CAB & THE DOOR. THE VEHICLE WAS PARKED ON ADOWN SLOPE UNEVEN TERRAIN. THE INJ WENT TO THE M INE CLINIC & FOLLOWED UP AT KAYENTA CLINIC, HE HAD A SMALL FRACTURE ON HIS LEFT MIDDLE FINGER

THE INJURED WAS POUNDING IN STAKES FOR SILT FENCE USING A 20 LB SLEDGE HAMMER. HIS FINGER GOT CAUGHT BETWEEN THE STAKE AND THE SLEDGE HAMMER HANDLE. THE TIP OF THE RIGHT INDEX FINGER WAS HIT C AUSING IT TO BREAK THE SKIN.

EE WENT INTO SOLUTION TANK TO REPLACE OLD FLOAT. HE STEPPED OFF LADDER TO PUT FOOT INTO SOLUTION. SOLUTION WAS DEEPER THAN HE THOUGHT & HE SLIGHTLY FELT HOT SOLUTION. AS HE STARTED TO CLIMB LA DDER, HIS FOOT SLIPPED AND HE PUT BOTH FEE INTO SOLUTION THAT RAN INTO HIS BOOTS. HE CLIMBED TO THE TOP OF SILO TOOK OFF HIS BOOTS, SPRAYED THEM WITH COLD WATER, APPLIED ANTISEPTIC & GAUZE TH

INJURED WAS HELPING W/MOVE OF OFFICE FURNITURE& HARDWARE WHEN HE FELT LOWER BACK PAIN.

INJURED WAS CLEANING THE COMMUTATOR ON THE PROPEL MOTOR WHEN IT FLASH CAUSING HEAT WHICH BURNT HIS L HAND.

INJURED WAS STRIKING A EQUALIZER SIDE PIN WITH A HAMMER ON A DOZER, WHEN A PIECE OF METAL BROKE OFF AND STRUCK HIM ON HIS L UPPER LEG.

OEPRATOR WAS DISMOUNTIN GOFF THE TRUCK (F) BUMPER, MOUNTING STEPS WHEN HE SLIPPED AND FELL TO THE GROUND CAUSING A SEVERE SPRAIN TO RIGHT ANKLE.

THE MECHANIC WAS REPAIRING A RADIATOR HOUSING ASSEMBLY WHEN HIS HAND SLIPPED AGAINST A PUTT KNIFE CAUSING A LACERATION TO HIS LEFT INDEX FINGER

THE EE SOUGHT AND RECEIVED MEDICAL TREATMENT FOR A CHRONIC AILMENT ON HIS KNEE THAT REQUIRED ARTHROSCOPIC SURGERY.

THE WELDER WAS LIFTING THE SOCKET WEDGE TO GUIDE IT INTO THE SOCKET OPENING WHEN HE PINCHED HIS LEFT MIDDLE FINGER BETWEEN THE SOCKET AND THE WEDGE

HE WAS DRIVING ON A MUDDY ROAD WHEN HE LOST CONTROL OF HIS TRK., WHEN HE APPLIED HIS BRAKES. THE TRUCK ROTATED 180 DEGREES AND SLID OFF THE ROAD AND TURNED OVER ON ITS LEFT SIDE.

THE INJURED WAS GRINDING A PIECE OF METAL WHEN HE TILTED HIS HEAD BACK TO AVOID LOOSING HIS SAFETY GLASSES, WHICH CAUSED HIM TO LOSE HIS ATTENTION OF THE TASK. APPARENTLY HE LOST HIS GRIB OF T HE METAL AND PINCHED HIS 12 INDEX FINGER BETWEENTHE WHEEL AND THE TOOL REST.

THE INJURED WAS USING A PRY BAR TO REMOVE A WEDGE FROM THE DRAGLILNE ROPE ASSEMBLY. THE TENSION OF THE WEDGE & THE ROPE CAUSED HIM TO FALL AGAINST THE DOZER BLADE. THE FALL CAUSED HIM TO HAVE HIS LEFT THUMB CAUGHT BETWEEN THE PRY BAR & THE DOZER BLADE CAUSING INJURY.

VICTIM WAS ASSISTING IN REPLACING A DRAGLINE BUCKET TRUNION PIN WHEN HIS LEFT HAND GLOVE GOT CAUGHT ON ROUGH EDGE OF PIN HEAD SMASHING HIS FINGER.

THE INJURED HAD HIS LEFT HAND ON OR NEAR THE PULLDOWN CHAIN ON 802 DRILL WHEN IT WAS STARTED UP. HIS 4 AND 5 FINGERS WERE CAUGHT BETWEEN THE CHAIN AND SPROCKET, CUTTING THOSE FINGERS.

WHILE STANDING ON FLAT BED TRAILER, METAL SIDE STAKE FELL AND STRUCK KNEE.

THE INJURED AND HIS CO-WORKER LIFTED A C3"X32" CARRIER BEARING BOLT WITH A PRY BAR TO TIGHHTEN THE BOLT NUT. THE PRY BAR SLIPEPED OFF AND HTE BOLT DROPPED ON HIS RIGHT LITTLE FINGER.

THE INJURED WAS GRINDING INSIDE A BORE WHEN THE ROATING DISC CONTRACTED THE EDGE. IT THREW THE GRINDER TOWARDS HIM. THE DISC CONTACTED HIS NECK CAUSIN GA SUPERFICIAL CUT.

EE REPORTED CHEST PAIN, SHORTNESS OF BREATH AFTER EATING HIS LUNCH AT THE JOB SITE.

THE MECHANIC WAS REPAIRING THE ELECTRICAL WIRIGN ON THE REAR SECTION OF THE WATER TRUCK USING A PORTABLE LADDER TO PERFORM THE TASK. WHEN HE STARTED TO CLIMB DOWN, HE SHIFTED HIS WEIGHT ONTO A LADDER WHICH FELL WITH HIM. THE LADDER FELL ON HIS LEFT ANKLE AND FOOT.

INJURED WAS ASSISTING THE CREW CHAIN HOIST AND MOVE THE HIGH VOLTAGE COLLECTOR RING AND FRAME INTO PLACE. WHILE USING THE PRY BAR TO LIFT THE LOWER FRAME INTO PLACE, THE BAR SLIPPED, TILTING T HE TOP FRAME WHICH FELL ON HIS LEFT HAND, BREAKING HIS INDEX AND MIDDLE FINGERS.

THE EE WAS REMOVING THE HYRAULIC RAM OFF THE DRAGLINE SHOE, UNDER THE WALKING CAM WHEN IS LEFT LITTLE FINGER WAS CAUGHT BETWEEN HYDRAULIC RAM& THE SHOE DECK.

INJURED WAS ROLLING OUT A POWER CABLE FROM A CABLE REELER WITH A LOG-SKIDDER WHEN HE WAS INJURED HE PLACED HIS (R) HAND ON A HANDHOLD WHICH WAS LOCATED NEXT TO THE OPEN CAB DOOR. THE DOOR SLAM MED AND CLOSED AGAINST HIS (R) SMALL FINGER.

INJURED BUFFING SOME OLD PATCH STRIPS ON A BELT CONVEYOR WITH BOTH OF HIS LEGS APART. HE WAS BUFFING BETWEEN HIS LEGS WHILE SITTING ON THE BELT, WHEN THE FUBBER CAUGHT A LOOSE STRIP IT THREW T HE BUFFER AGAINST HIS LEF LOWER LEG.

INJURED WAS GOING TO STEP OVER A SWING CASE WHENHE TRIPPED ON HIS LEFT SHOE STRING AND FELL ONTOTHE SWING CASE HITTING HIS HEAD ON THE BRAKE GUARD.

EE WAS UNSING A STITCHER FASTENING PATCH ONTO BELT WHEN HE STRAINED HIS LOWER BACK. HE WAS ON AN UPWARD PUSH WITHOUT HAVING COMPLETE CONTROL OF THE EQUIPMENT.

INJURED WAS DRIVING UP A RAMP WITH A LOAD OF ROCKS WHEN THE ENGINE STALLED OUT CAUSING THE TRUCKTO ROLL BACKWARDS. THE LEFT REAR WHEELS CAME UPON A SPOIL TILTING THE TRUCK BED AND ROLLED IT OV ER ON IT'S RIGHT SIDE.

THE INJURED AND HIS CO-WORKER WERE PLACING A SHEET OF MEAL (48"X36"X3/8") INTO A METAL BUILDING WHEN IT SLIPPED OUT AND FELL ON HIS RIGHT FOOT. THE METAL SLIPPED OUT WHEN HIS CO-WORKER TRIED TO HAMMER IT IN ON THE UPPER END OF THE SHEET OF PLATE.

A DIAGNOSED PRE EXISTING ILLNESS. AN ACUTE CEILUTLITIS DEVELOPED INTO AN INFLAMMATION AND INFECTION ON THE RIGHT CALF AFTER AN OLD WOUND REOPENED.

THE INJURED WAS SECURING THE BLOCK OF THE OVERHEAD CRANE ONTO AN I WHICH WAS WELDED TO THE FLOORWHEN HE ACCIDENTALLY PUSHED THE CONTROL BUTTON WITH HIS HAND. THE MOVING CABLE OF THE CRANE CAUG HT HIS LEFT LITTLE FINGER AGAINST THE BLOCK CAUSING A CUT.

THE INJURED WAS ASSISTING A DRILLER ALIGNING A DRILL STEM WHEN THE PRESSURIZED AIR WAS RELEASED NEAR HIS R EAR. THE PRESSURE BLAST FROM THE AIRLINE EMITTED INTO THIS R EAR CAUSING HEARING LOSS .

EE WAS ASSISTING 2 MECHANICS INSTALL A BRAKE SHOE ON THE REAR TRAILER WHEN IT SLIPPED OUT OF THE DRUM AREA. EE RT. HAND WAS CAUGHT BY A SHARP LOCK PLATE AS THE BRAKE SHOE FELL CAUSING A CUT IN THE PALM OF HIS HAND.

INJURED WAS EXITING OUT OF THE REAR OF A CAR PRIOR TO SHIFT WHEN HIS LEFT LEG BECAME ENTANGLED WITH THE SEAT BELT STRAP. THE SEAT BELT PULLED HIM BACK AND FELL ON HIS RIGHT ANKLE CAUSING A FRA CTURE.

THE INJURED WAS CLOSING THE DOZER CAB DOOR FROM THE INSIDE WITH HIS LEFT HAND WHEN HIS [R] MIDDLE FINGER WAS CAUGHT BETWEEN THE [R] DOOR & THE DOOR JAMB. THE INJURED SUSTAINED A SEVERE CUT ON HIS MIDDLE FINGER.

ILLNESS DUE TO CONTINUAL EXPOSURE OF OPERATING TRACTOR CAUSING SORENESS IN [R] ELBOW - CONTINUALARTHRITIC CONDITION.

INJURED WAS REPLACING TRANSMISSION ON HAUL TRUCK WHEN THE LIFTING EYE BOLT BROKE AND THE BROKEN PIECE HIT HIM ON HIS FOREHEAD.

THE INJURY OCCURRED WHEN HE REMOVING A FULL NITROGEN BOTTLE OUT OF THE RACK. THE INJURED LIFTED THE, APPROX. 2" TO GET IT OVER THE RACK LIP WHEN HE SUSTAINED THE INJURY.

INJURED WAS SITTING NEXT TO A DESDK WHEN THE CHAIR W/WHEELS ROLLED OUT FROM UNDERNEATH HIM CAUSING HIM TO FALL ONTO THE FLOOR IN A SITTING POSITION

THE INJURED WAS LIFTING A CUT METAL PLATE 4 1/2 X 1 OUT OF A COMPARTMENT TO COVER AN EXPOSED SWING BOSS BEARING HOLE WHEN THE INJURY OCCURRED. THE INJURED PUSHED ON THE PLATE TO GET HIS FINGER S BETWEEN THE PLATE AND THE COMPARTMENT STRUCTURE WHEN THE PLATE FELL OVER CATCHING HIS R LITTLE FINGER.

THE INJURED BACKED THE TRUCK NEXT TO THE DRAGLINE SHOE TO PICK UP A LOAD, WHEN THE MACHINE BEGAN TO SWING. HE JUMPED BACK INTO THE CAB TO MOVE IT, BUT THE DRAGLINE CAUGHT CAUGHT THE TRUCK AND TIPPED IT ONTO ITS RIGHT SIDE. THE UNBELTED INJURED FELL TO THE LOWER SIDE OF CAB AND THE DRAGLINE JOLTED THE TRUCK MOMENTARILY.

THE INJURED WAS ASSISTING THE ELECTRICANS MOVE THE TRAILING CABLE TO THE DRAGLINE EARLIER IN THE SHIFT BUT TASK CAUSED SEVERE PAIN A COUPLE HOURS LATER THE INJURED SUSTAINED A RIB SPRAIN/STRAI N AROUND HIS CHEST AREA THE CABLE WAS NOT ENTERNIZED TO THE MACHINE

THE INJURED WAS CLEANING ON THE VIBRATOR FLOOR, WHERE THE TRAPDOOR WAS OPEN AND THE INJURED TRIPPED AND FELL THRU THE OPENING ONTO THE NEXT LOWER LEVEL. THE OPENING (3 X 4) WAS ROPED-OFF TO PREVENT ENTRY BUT THE INJURED WENT PAST THE ROPED OFF AREA WHEN HE FELL.
