This day was raining and the ground was muddy, EE was helping another employee with the Cone Crusher QH-331, on a crusher jam. When EE was getting off the Crusher proceeded to step off the Crusher through the ladder, ladder was wet and slip while stepping down, Fail in EE's left foot.
