EE WAS USING A PRY BAR ATTEMPTING TO ALIGN UPPER ASSEMBLY OF CRUSHER WHEN BOLTS SHEARED OFF THROWING HIM 8-10- FEET TO THE GROUND. EE HAD JUS CHANGED LINERS IN CRUSHER.
EMPLOYEE GOT OFF OF RAM-HOE TO HELP OTHER EMPLOYEE MOVE MATERIALS THROUGH FEEDER. LOST HIS BALANCE. RIGHT LEG WENT THROUGH GRATE OF FEEDER.
