WHILE CLIMBING DOWN FROM DOZER EMPL'S FOOT SLIPPED ON TRACK & FELL STRIKING BACK. ACCIDENT OCCURED ON 12/4/90 BUT EMPL DID NOT START MISSING WORK UNTIL 6/19/91,
WAS MAKING SAFETY BENCH FOR DRAG, GOT OUT ON TRACK AND ROCK CAME DOWN OFF HIGHWALL AND HIT HIM ON RIGHT LEG.
WHILE REMOVING BACK OF BELLY PAN FROM CAT 980 C, HE REMOVED 3 OF 4 BOLTS AND THE 4TH BOLT BROKE, LETTING THE PAN FALL ON HIS FOOT. THIS CAUSED HIS GREAT TOE ON HIS RIGHT FOOT TO BREAK.
