BUILT GRAVEL PILE UNDER SHAKER ON SCREENING PLANT IN ORDER TO HAVE FLAT SURFACE TO WORK ON. I BACKED THE SERVICE TRUCK UP TO THE PILE. I CARRIED SHAFT UP THE SIDE OF THE PILE AND MY LEFT LEG S LIPPED. BY THE TIME I WAS DONE WITH JOB, MY GROIN WAS VERY SORE. THIS HAPPENED ON 12/14/98 ABOUT 10 AM. THEN ON 1/25/99 AT ABOUT 7 AM MY LEFT FOOT SLIPPED.
