Vulcan Construction Materials, LLC · Struck by falling object

Removing an umbrella stand from the back of the service truck. Set stand down on uneven ground, Stand fell striking EE's gloved Right inner thumb and palm. Suspected fracture confirmed by Ortho on 9-24-2025.

Injured employee was cleaning plant floor with shovel when he slipped as he was pushing shovel with left foot. Injured fell and landed on buttock with right leg underneath him. Injured did not report until next day. Injured suffered groin pull and is on light duty.

Injured was helping startup 6" waterline from Ground Well. After repairs to hose connection, plugged line burst open and rubber water line struck injured in Left leg below his knee. Injured was diagnosed with contusion to leg with no fractures. Injured was sent home for two days. Back to work on Monday.

While using a steel pry bar to dislodge rock from jaw crusher, Injured employee slammed his right hand into side of crusher wall, causing fracture to tip of right index finger. Crusher and conveyors were locked out and Injured was wearing gloves.

The report is late due to injured not reporting injury timely, and also due to the litigation between dr and insur. on compensability and past medical surguries in same abdomen area.

INJURED WAS CHANGING OUT SHAKER SCREEN ON SHAKER WHEN HE SLIPPED ON CORSS BEAM SUPPORTING SHAKERINJURED FELL APPROX. 8' DOWN GRAVEL CHUTE. CHUTE WAS EXPSED BY ROLLING BACK OF ROLL-AWAY DOOR IN ORDER TO GET SCREEN OUT OF SHAKER DECK. INJUREDSUFFERED MILD FRACTURE THAT REQUIRED AN AIR CAST. INJURED WILL NOT LOSE ANY TIME WITH WILL RETURN TO WORK AS A SCLAE HOUSE ATTENDENT.
