Plant employee was using a utility knife to cut holes in skirt rubber for installation on screen shaker. During the cutting process, the tip of the blade broke, causing the knife to slip and resulting in a laceration to the employee's left arm above the wrist. The employee received 6 staples for the injury and placed on light duty for seven days.

Employee was removing angle brace from catwalk structure during its fabrication. Employee failed to seek assistance before removing final weld, however, employee proceeded to remove last weld causing angle to fall to the ground and rebounded, and struck left foot steel toe area causing injury to left big toe. Possible fracture.

Employee was starting to do oil change on skid steer when EE started feeling chest pain & shortness of breath. Co-worker took EE to scale house to report issue and call 911. Ambulance came and transported EE to hospital. EE was checked and released to see primary doctor.
Employee fainted, spouse showed up and says this happens often when EE forgets EE's medication, so the spouse gave EE the medicine and employee began to feel better. EE suffers from high blood pressure and diabetes. We are unsure what medication was given. 911 was called, employee did not want to go to hospital but Company stated it was better if EE went to get evaluated.
Employee was using a prybar to loosen build up material from feeder due to conveyor ripping, EE was using a stabbing motion to release material using prybar and tool shifted hitting pinky finger against guard and Prybar. Employee fractured tip of left 5th digit finger and got sutures.

Employee was shoveling spillage next to conveyor. Employee was struck on left hand with rock that came off conveyor screen. Employee did not report incident that they says occurred at 1:30PM employee reported to work following day and reported incident to supervisor, in which they requested medical attention.
Employee was checking tension on drive belts from caliche plant shaker. Employee locked out equipment and grabbed ahold of belts with right hand and when they pulled the sheave it shifted, catching employees hand between belt and sheave. Employee went to clinic where it was determined with X-rays of fractured index finger.

Employee was adjusting the forks on forklift, when employee moved them towards the middle one of the forks remained in the up position. Employee with left hand grabbed the chassis and with right hand pushed the fork so it could return to original position, while doing so they didn't remove their hand and hit left thumb. Employee fractured finger.

Employee was turning a wrench to tighten a u bolt on spray bar, the wrench came off the bolt head causing the finger to get pinched between hand and screen

EE is a loader operator, as EE was climbing down from loader EE felt a pulling pain in the leg. As EE got off and proceeded to walk, that is when EE's leg gave out.
Employee was loosening nut on bolt and when it became loose the employee lost balance and fell back hitting EE's back on electrical box.
Employee was working on Texas Crusher when EE felt some abdominal pain.
Employee was working by cone crusher and a stone / rock hit employee on the back.
As employee proceeded to check a feeder EE stepped on uneven ground and stepped wrong and EE's knee popped.

Employee was struck by fallen object, asked if OK and was OK. Later that morning employee complained of some neck pain and send to clinic where employee was later released. Back to full duty on 12/7/2018

EE was cutting a conveyor belt at sand & gravel plant to get it repaired the knife ee was using accidentally slipped and cut ee in the left leg. EE was take to clinic where ee was treated and released to return to work on modified duty.

EE#1 was inside an Impact Crusher along with EE#2 & EE#3 changing impact crusher plates. EE#2 & EE#3 had finished the work and exited the machine but when EE#1 was about to exit the machine, he felt the machine move / rotate. EE#1 attempted to hold down or stop the metal bar from moving when he slipped, twisting his left ankle.
