Miner was bolting up hog panel in the heading. When the miner started drilling the panel shifted causing the bolt driver and plate to come loose and fall towards the miners left forearm where the roof plate contacted EE's left forearm above the cuff of EE's glove resulting in a "T" laceration requiring sutures.

Miner was pulling drill out of hole when EE lost EE's balance and fell. As miner fell EE's middle finger on left hand became caught between the machine and leg pinch point. This caused a laceration to the left 3rd finger. Miner was taken to clinic for fingernail removal at nail bed requiring 7 sutures.
Miner had completed sounding, mechanical and hand scaling of the back and ribs and was preparing to begin the bolting process. As the miner turned towards the machine a piece of rock bounced off their hard hat and shoulder before falling and contacting their left ankle resulting in a compound fracture and a fractured neck vertebrae.

Miner was contacted on the right lower torso/rib area with the airline spud and wingnut resulting in fracture to the ribs. The airline was caught on the cab of the mucker and was not seen by the miner until it had pulled tight and released striking the miner.
Miner had completed drilling hole in ore face, their right hand slipped from the D handle as the hammer cammed over and pinched right middle finger between the hammer and the leg connection point.

EE was assisting with connecting air compressor to p/u truck. EE attempted to move the tongue of the air compressor to the hitch of the p/u truck when EE felt a snap in their left shoulder.

EMPLOYEE WAS TAKING ASHOWER USING LIQUID DISHWASHING DETEREGENT, GOT IT IN HIS EYES. DISCOMFORT, DOCTOR USED EYE DROPS.
EMPLOYEE WAS CUTTING ANGLE IRON BRACING FROM AN OVERHEAD TRAVLING HOIST BEAM, WHICH WAS LAYING ON THE GROUND, WHEN BRACING WAS REMOVED, THE HOIST BEAM ROLLED OVER, STRIKING EMPLOYEE ON INSTEP OF LEFT FOOT. CONTUSION TO INSTEP OF LEFT FOOT.
EMPL WAS INSTALLING ROCKBOLTS WITH PLATES THROUGH CHAIN LINK FENCING. HE HAD DRILLED BOLT HOLE & WAS USING BOLT TO STRETCH FENCING WHEN BOLT SLIPPED ALLOWING PLATE TO STRIKE INJURED ON RIGHT L OWER LIP RESULTING IN 3/4" LACERATION WHICH DID NOT CUT THROUGH LIP.
EMPLOYEE WAS DRILLING, WITH LEFT HAND ON HANDLE OF DRILL, SMALL ROCK FELL FROM BACK STRIKING MIDDLE FINGER OF LEFT HAND.
