Laoc Corporation · Struck by rolling or sliding object

Shot was hooked up to cast. 42 + 67 ms between rows. Held 12' stem on inside of shot, 14' on outside. Shot climbed approx 200' high then went outward hitting equip. & pickup. Face was checked with tape. Holes were checked as loaded. No problem with any holes, all holes looked solid. Employee was hit with flyrock while observing shot.
