THE PLANT MANAGER WAS ASSISTING ELECTRICAL CONTRACTOR LOCATE A BROKEN WIRE TO THE BIN VIBRATORS. HE WAS ASKED TO ENTER THE POWER HOUSE AND MAKE A JUMPER WIRE TO JUMP ACROSS TWO POLES ON THE VI BRATOR SWITCH BOX. WHILE STRIPPING THE ENDS, ONE INADVERTENTLY CONTACTED TWO LEAD POSTS OF ANOTHER SWITCH BOX NEARBY, CAUSING A DEAD SHORT AND BURNING HIS HAND.

EE DEVELOPED WHAT APPEARED TO BE A SMALL HERNIA. NO SINGLE EVENT COULD BE IDENTIFIED THAT CAUSED THIS CONDITION. EE SHOVELS ON A DAILY BASIS AND ASSISTS LOADING OF SHOT ON OCCASION. MEDICAL EX AM INDICATED SMALL HERNIA, SURGERY IS SCHEDULED FOR 7-7-03. ESTIMATED TIME OFF OF 1 WK, WITH LIGHT DUTY AFTERWARDS.

EE WAS CLIMBING DOWN THE LADDER OF THE FRONTEND LOADER AFTER PUTTING IT INTO THE POLE BUILDING. APPARENTLY HE MISSED THE LAST STEP AND FELL. WE BELIEVE HE FELL ON A SAWHORSE AND BROKE SEVER AL RIBS. EE SEEMED A LITTLE CLOUDED ON EXACTLY HOW IT HAPPENED. THERE WAS NO APPARENT HEAD INJURY.
