Luck Stone Corporation · Struck by flying object

Employee was working from man basket, removing messenger wire for ventilation tube, during this process a piece of metal debris got into eye. Employee noticed some irritation later in shift but didn't start hurting till next day. Employee went to local clinic and got piece of debris removed from eye.

Employee was scaling to mark out location, while scaling overhead, a piece of rock measuring 1'x2'x1' came down and hit employee on side of hardhat, cut neck and hit hand on way down to basket.

Employee was clearing obstruction out of end of shotcrete hose. Took safety glasses off to see in the hose when the obstruction came out, striking the employee in the face with shotcrete. Resulted in chipped tooth and flushing of eyes with water.
