A piece of rock fell hitting employee in back and knocked him into the bolter as he was on his knees bolting top causing laceration to forehead. Sutures involved.

EE WAS MOVING CABLE OUT OF ROADWAY ON 5/21/04 WHEN HE NOTICED HE WAS HAVING PAIN IN HIS BACK. ON 6/3/04 EE WAS TOLD BY A DOCTOR TO TAKE TIME OFF FROM WORK BECAUSE HE HAD A BULGING DISC IN HIS BACK.
EE WHILE LOADING BOLTS & PLATES ON ROOF BOLT MACHINE SLIP AND FELL ON SCOOP BUCKET HIT SHOULDER AND BRUISED HIS SHOULDER.

EMPLOYEE WAS HELPING MECHANIC LOAD MOTOR TO TAKE INSIDE. HE CAUGHT TIP END OF HIS RING FINGER UNDER EDGE OF MOTOR. IT MASHED END OF FINGER CAUSING NAIL TO COME OFF. HE HAD GLOVES ON.
EE TRIMMED SKIRT ON STACKER BELT WITH A SHARP POCKET KNIFE. IT SLIPPED AND CUT HIS RIGHT POINTER FINGER. IT TOOK TEN STITCHES.
EE SLIPPED AND FELL AND HIT SHOULDER ON GROUND AND HURT MUSCLE IN RIGHT SHOULDER.
AFTER LOADING CABLE EE STARTED TO SIT DOWN AND NOTICE LOSS OF FELLING IN RIGHT ARM. THOUGHT HE MIGHT BE HAVING HEART PROBLEM. HE TOOK NITRO-GLYCERIN PILL AND WENT TO HOSPITAL. THEY THOUGHT HE MIGHT HAVE A NERVE PINCHED IN HIS NECK. HE HAD BEEN LOADING 1/0 CABLE.

EMPLOYEE WAS ASKED TO BRING A CAN OF OIL TO ROOF BOLTER AFTER PUTTING OIL IN ROOF BOLTER OPERATOR OF ROOFBOLTER STARTED BACK TO BOLT TOP. HE DIDN'T SEE EMPLOYEE HAD LAID HIS HAND ON BOLTER, WH EN HE SWUNG HEAD TO ONE SIDE IT CAUGHT EMPLOYEE'S FINGER AGAINST FRAME OF BOLTER.
