Employee was removing the bucket from front end loader and while removing the pins from the bucket, EE pinched EE's left fourth finger in between pins and bucket causing laceration to left fourth finger.

Pulling out stuck utility service truck with a backhoe and sustained wheel damage to backhoe.

Employee was shoveling mud near the conveyer belt. Employee lifted shovel full of mud and turned. While lifting and twisting, employee felt back pop and felt pain.

Metal bar broke loose and struck employee in the knee.

Employee was holding the scrapper when co-worker swung the sledge hammer and it slipped striking employee on right hand.

IE and another EE were changing out a rubber skirt on BC27 (conveyor belt), IE pushed it and it went on top of the belt, IE walked around the pulley to get it off the belt, the other EE continued pushing it and caught IE's finger.

As EE was getting off the loader, foot slipped on wet ladder/step as he held himself up with right arm to avoid falling off the loader he felt pain on his right arm.

Rock stuck between blade and liner of dozer; employee used sledge hammer to dislodge rock. Rock came out and hit his left leg, cutting it.

Shoveling on BC19 clearing return roller, shovel hit the return roller and popped up, employees hand was between the shovel and the conveyor and ripped his left pinky finger nail off.

While employee was setting up to wash off wet side slab he bumped his head on conveyor causing laceration to right eyebrow.

EE was using a pry tool to lift flexcos off a conveyor belt, tool slipped and hit him on his lower lip resulting in laceration of lower lip and fracture of 3 teeth.

Employee said he had been feeling pain in stomach area since yesterday - at 10:40 am employee was approaching an EE Quarry Foreman when he felt something popped in stomach and curled down in p ain. Employee was taken by EMT to Antelope Valley Hospital. Diagnois: Hernia.

Employee states that while driving a loader, hit a rock or bump and jarred the loader and hurt his back.

One hour after employee was moving a gear box that was hanging on the forklift he felt pain in his lower back.

Employee was sent to High Desert Medical Group for yearly hearing exam on 2/1/10 doctor requested to see employee again for a follow up. Employee was seen on 3/22/10 and was recommended to see a specialist for moderate hearing lost. Employee claims the hearing lost began on 3/5/07.

Rock discharge from crusher, hit conveyor belt and bounced striking employee on side of hard hat.

EE SHOULD HAVE BEEN STANDING ON THE GROUND. INJURY UNKNOWN, TEST RESULTS NOT CONCLUSIVE. IMPAIRMENT, UNKNOWN PER DOCTOR ACTIVITIES AS TOLERATED

EE WAS LOOSENING TAIL PULLEY NUT, HAD HIS HAND ON THE NUT, ANOTHER EE SAT DOWN ON THE BELT CAUSING THE TAIL PULLEY TO SLIDE FORWARD, CATCHING HIS HAND.

EMPLOYEE WAS CUTTING BOLT HOLES IN THE BELT WITH A UTILITY KNIFE. THE KNIFE SLIPPED CUTTING HIS LEFT HAND.

EE FELL ON THE SCREW DECK AND LANDED ON HIS BACK.

EE WAS ASSISTING IN LAYING 0PIPE IN A 3-4 FOOT TRENCH. THE TRENCH COLLAPSED AND BURIED EE TO JUST BELOW THE KNEE.

EE WAS ASSISTING IN LAYING PIPE IN A 3-4 FOOT TRENCH. THE TRENCH COLLAPSED AND BURIED EE UP TO BELOW HIS HIPS.

RE-INSTALLING CATWALK. THE CATWALK HIT THE JAW AND THE LEG OF THE CATWALK BROKE OFF AND LANDED ON EE'S FOOT.

EE TRIED TO MOVE A 24" PIPE ALON

PULLING CABLE IT GOT STUCK IN SPINDLE OF WINCH GRIP CAME OFF HANDLE OF WINCH EE FELL BACK-CAUGHT HAND BETWEEN DOOR OF TRUCK AND SELF.

WORKING WITH LOADER. STEPPED OUT OF EXIT DOOR. EARTH WAS LOOSE AND GAVE WAY CAUSING BODY TO THRUST TO EE'S LEFT SIDE AND FALL DOWN. RIGHT LEG WAS CAUGHT IN DIRT.

CUTTING METAL OUT OF CONE CRUSHER. WHILE USING WELDER, GOT A FLASH BURN TO EYES.

EMPLOYEE WAS STANDING ON THE FRAME OF THE JAW CRUSHER ASSISTING WITH THE INSTALLATION, LOST HIS FOOTING, SLIPPED AND FELL TO THE GROUND, APPROX. 10 FT.

WORKING INCLINED SHAKER SLIPPED TRYING TO CATCH FROM FALLING R FOREARM CUT WITH KNIFE

EMPLOYEE ATTEMPTED TO LIFT A 8INX6FT CHANNEL (I BEAM) WHEN HE PULLED THE MUSCLES IN HIS STOMACH & HIS TESTES

EMPLOYEE REPRTED THAT AT APPROX 4 PM WHILE OPERATING A LOADER HE HIT A HOLE AND HURT HIS BACK.

EMPLOYEE WAS ON TOP OF THE WET SCREEN PLANT WELDING EMPLOYEE WAS DISENGAGING A WELDING CABLE WHEN THE CABLE SUDDENLY WHIPPED AROUND THE TOP RAIL CATCHING HIS LEFT THUMB

WHILE EMPLOYEE 4AS USING THE CUTTING TORCH ON A SECTION OF THE JAW CRUSHER A LARGE SPARK AND /OR A PIECE OF METAL BURNED A HOLE IN THE OXYFEN HOSE IT RUPTURED APPROX 1 INCH BAKC FROM THE TORCH HANDLE 3

EMPLOYEE WAS AT7EMPTING TO TIGHTEN JAW ON JAW CRUSHER EMPLOYEE STEPPED DOWN FROM MAIN FRAME OF PORTABLE CRUSHER ONTO TAILGATE OF A CO.VEHICLE TAILGATE SUDDENLY OPENED EMPLOYEE LOST BALANCE & RT.HAND(R-RING)3FINGER MADE CONTACT WITH FLY WHEEL DRIVE

EMPLOYEE WAS CL3MBING A LADDER WITH A 50 LB. PIECE OF STEEL ON HIS SHOULDER. AS THE EMPLOYEE WAS ASCENDING THE LADDER HE TWISTED HIS RIGHT KNEE.
