Stepped off of forklift onto uneven ground causing a minor fracture to left foot injured employee is on restricted duty

While cleaning a chute with water the individual had wet high alkali material get behind EE's safety glasses and into EE's eyes causing minor chemical burns

While inspecting a port on the tower the employee got hot dust in their face causing an eye abrasion and 2nd degree burns to face.

Employee was climbing into the cab of the bulldozer EE operates when EE's foot slipped off the step and EE fell injuring EE's ankle in the process.

While using an air lance to clear a chute the employee got clinker dust into EE's eyes. EE's eyes were flushed and EE was administered an antibiotic eye drop medicine.

Employee was grinding a beam when it grabbed on the beam and the hand holding the handle slipped off. The grinder slipped back into their wrist as they came off the trigger with their other hand. The disc caught their left forearm and required 4 stitches. EE was wearing proper PPE and all guards were in place.

While changing rolls employee had a piece of clinker dust get behind EE's safety glasses and in EE's eye. EE was given antibiotic eye drops after removal of particle on 03/03/2025

Employee was cutting a plastic bucket, the knife slipped off the bucket and cut their hand

Employee was cleaning around pulper pit. The steel floor covering the pit Flexed and slipped off of the support causing the employee to fall. EE caught self with both arms. Resulting in shoulder injury. Procedure took place on 11/07/2024 resulting in restricted duty but the restricted duty began on 11/11/2024

Piece of shafting fell from lathe causing a fracture to right foot metatarsal bone.

Individual was using a rod to change valve position. The valve was difficult to move so they jerked on the rod which rotated and struck them in the face above the left eyebrow causing a laceration.

While emptying the fine coal bin some hot material in the bottom of the fine coal bin became exposed and ignited float coal dust within the bin.

Employee was switching a valve and finger got smashed between valve handle and cement line causing a fracture to finger

Employee was striking a stuck carry roll on a conveyor. The roll started to spin and pulled employee's hammer in to it. This drew employee's hand across a sharp piece of metal on the framework

While beating on a chute the employees hammer contacted an airline slightly above and behind EE causing it to be redirected which caused it to hit the left thumb of EE's off hand. This created a laceration which required a few stitches to close up.

After prolonged use of a sledge hammer the EE started having pain in arm. Due to inflammation from swinging the sledge hammer EE experienced a pinched nerve in right shoulder.

2 miners were stuck in the tower elevator when a safety switch became loose allowing the circuit to open which stopped the car

While moving 515V tripper employee got foot in pinch point between roller and rail.

Injured employee was accessing tanker, when gangway shifted employee stepped through the gap between the gangway and the tanker twisting a knee.

We believe that due to the age of the motor control center and the brittleness of the wiring the work they were doing may have caused a wire to short out and cause the fire. The motor control center has been de-energized and a new one has been ordered for installation.

Smoke was seen coming from the coal tunnel. Coal was removed from above the feeder below and coal in the feeder was run out onto the ground. There were no flames, only smoke and smoldering coal were observed. Called EE once situation was resolved to ask if it was reportable or not since there was no flame to extinguish and EE said to go ahead and report it.

Employee was using a hydraulic puller to remove a gear from a shaft. EE had a hand on the body of the puller. The puller came loose from the gear and EE's finger was momentarily caught between the arm and the body of the puller causing a fracture to the little finger.

While opening a door a gust of wind swept up debris and something got into right eye. Flushed eye and went back to work. Continued to have redness but no discomfort. Sent to company eye doctor on June 22 for examination where they realized a piece of debris on the eye that needed removal.

Miner was trying to get silo to feed and cement eventually began to flow at a fast rate and got in the right eye of the employee.

Individual was climbing a fixed ladder and lost EE's grip or footing and fell approximately 6 feet to the ground below fracturing the humerus of EE's right arm and dislocating EE's right shoulder. Employee is on restricted duty.

Employee stepped down from dozer and foot slipped on unconsolidated material causing employee to injure knee.

Employee was placing a barricade around some I beams that had been removed when EE walked into the end of one of them that had a sharp edge and cut leg just above EE's boot.

Employee fell through sheet metal roof and was suspended above an operational kiln. Injuries were due to the extreme heat, not from the fall. Burns were primarily to the buttocks and backs of legs.

Employee was removing welding lead from a container. EE was pulling it out from storage and setting on a pallet for relocation to the work area. Did not feel well, when reporting to work the next day said had soreness in groin. Took EE to clinic for evaluation and diagnosed as groin strain

The employee was on the third rung of a 6'-0" ladder using a harness with a retractable lanyard. The employee was tied off and using a plasma cutter to cut out grizzley bars. While cutting, slag went down the employee's shirt, and EE fell from the ladder. The platform the ladder was on had refractory on it, causing the employee to strain/sprain ankle when EE fell.

Individual was climbing ladder and lost EE's grip. EE jumped back landing on EE's feet. In doing so EE sprained EE's left knee and right ankle.

Employee was operating an off road forklift transporting some scrap metal, when EE hit a bump some debris on the load became airborne and entered the cab and got behind in between EE's face and safety glasses and into EE's eye.

Employee had just finished cleaning the riser duct and lifted face shield and dust fell from the frame of it behind employee's safety glasses into eye.

Employee injured right shoulder on 3-15-2020. Employee was terminated from employment on 5-9-2020 for reasons unrelated to the injury. During the course of EE's employment, the injury did not meet MSHA's part 50 reporting requirements. However, after termination the employee will be receiving surgery on the right shoulder on 7-10-2020.

EE was cleaning the riser with a pressure washer. When EE tilted EE's head to the left dust fell into EE's right eye.

EE was climbing a ladder to clean the burner pipe. As EE ascended dust fell from above getting in to EE's left eye.

Unloading ice melt off of a truck. EE twisted and felt a pain in the middle of EE's back.

Employee was trying to remove a chunk of cement from a valve when the chunk moved cement dust came out of the inspection cover and hit them in the face getting dust in their eyes.

EMPLOYEE WAS PUSHING A BLENDER INTO ITS CRADDLE, AND FELT A POP IN RIGHT SHOULDER. EMPLOYEE DISLOCATED R SHOULDER, WAS SET IN LOCAL ER AND WAS RETURN TO WORK ON RESTRICTED DUTY.

MINER SLIPPED ON MILL BALL.

Employee stepped backwards and tripped on a pallet lying on the floor behind self. Employee lost balance and fell on back and left side, folding left leg under self. Resulting in a sprain/strain to right knee. Employee was fitted for brace temporarily.

EMPLOYEE MIDDLE FINGER ON RIGHT HAND WAS BROKEN WHEN IT WAS CAUGHT UNDER A COUNTER WEIGHT.

MINER WAS CROSSING IN FRONT OF A PARKED TRUCK, SLIPPED AND FELL.

MINER WAS CLEANING BUILD UP OF FROZEN MATERIAL IN COAL HOPPER. EE WAS ABLE TO LOOSEN MATERIAL, BUT NOT TURN MATERIAL. EE REACHED INTO HOPPER AND TURNED MATERIAL. MATERIAL FELL AND STRUCK MINER IN R HAND BREAKING TWO FINGERS.

EMPLOYEE WAS THROWING TRASH IN ROLL OFF BOX, R HAND BECAME TRAPPED UNDER MATERIAL AND EDGE. EMPLOYEE JERKED HAND TO FREE IT, RESULTING IN FRACTURE TO METACARPAL FRACTURE TO THUMB.

EE was crossing a gantry from work platform to trailer. Gantry dropped slightly, causing EE to trip.

EE WAS LIFTING GRATING FROM GROUND LEVEL INTO CONTAINER BOXES. EE SPRAINED LOWER BACK ON R SIDE. DID RESULT IN LOST TIME INJURY.

EE'S LEFT HAND PINKY FINGER, SLAMMED IN DOOR. CAUSED LACERATION, REQUIRED 3 STITCHES.

EE was struck by metal shard, which penetrated the Left Forearm, initially was treated with first aid only on 08/17/2018. Later had the fragment surgically removed on 08/28/2018.

Miner suffered impact to left thumb.

Miner stepped into a floor opening, causing injury to R knee.

Employee was repositioning dust mask and lost grip. Mask popped against face causing dust to go under safety glasses and enter eye.

Employee splashed cement in eyes.

MINER TWISTED KNEE WHILE DISMOUNTING CRANE.

Miner struck middle finger on L hand. First Aid was administered, and EE was seen by Medical Professional.

Right foot became fowled by material buildup. Miner attempted to clean foot and strained R knee during material removal.

Employee lost balance while descending stairway.

Miner was attempting to open rail car to begin loading

Miner struck in L index finger by flying debris

Employee leaned beyond work zone while standing on ladder. Foot slipped off of wrung and employee fell striking the second lung on ladder.

Miner was using a sledgehammer to breakup debris. Felt something pop in their R shoulder area.

Miller was struck by bound door when energy was released

Truck Transport driver moved vehicle while employee was in Safety Cage on top of trailer. Employee suffered impact to R Lower leg calf.

EMPLOYEE TRIPPED AND FELL TO SAME SURFACE. EMPLOYEE HAVE RESENTLY HAD SURGERY ON LEFT KNEE. WHEN THE KNEE STRUCK THE SURFACE, THE SUTURES TORE

Employee was using a step ladder to paint from and when EE stepped down, EE missed bottom step.

Employee was closing tower elevator doors not using the grab handle and caught hand between elevator doors when closing, resulting in hand being injured small bone in back of hand fractured.

Employee was placing the truck wash system back into the designated place when the air hose which has a metal control handle at the end swung around and struck employee in EE's face chipping front tooth.

A contractor was welding in the pit of the solids building at the Alternative fuels site when an unplanned inundation of a gas ignited causing the welder to receive 2nd degree burns to face, back and arm.

Employee was transporting rollers on a cart up a ramp when one of the rollers came off cart striking EE's left leg causing a laceration that required six stitches.

Employee was cleaning side B K.O. Box. Employee was removing the lid after they had pried up the lid. Employee had both hands on both handles and pulled up. As they pulled up, felt a pop in right shoulder, then a hot shooting pain in shoulder.

Decision was made to empty fine coal bin to repair a leak. while being emptied coal off the walls fell onto hot coal inside causing the coal vent to activate.

EE was shaking/stirring 1 gallon paint can. Felt pull in his back.

ee smashed rt hand while training belt.

EE was dislodging large chunk of clinker with a pry bar and smashed his hand.

EE was carrying a bucket and slipped.

Employee was lifting hammers for the Crusher Dryer. He then felt a strain in his abdomen.

Employee was lifting lid from dust blender.

Employee injured right shoulder when pushing/pulling to remove burner lance from burner pipe.

Employee twisted left knee on grading of gangway.

Employees wrench slipped off bolt pinching his pinky finger between wrench and tie.

employee uncoupled hose from diaphragm. Liquid came out and got in his eye.

employee missed bottom rung of 4ft ladder. This caused injury to left leg.

Employees foot got caught in his lanyard. He tripped and hit his knee on the catwalk.

employee was driving to locate another employee for some chew tobacco, took eyes off of road hitting gangway framework with golf cart. employee is sore and cart is repairable

employee burnt arm while working on equipment.

employees was helping pull a conveyer belt together. Channel locks slipped off belt striking employee in eye. Safety glassed were worn.

Employee was struck on top of hard hat while cleaning tipping valves.

Employee says dust blew off from the brim of his hardhat and got in behind his face shield and safety glasses. Causing scratches to his eye and eyelid.

Employee missed rung when descending from ladder, injured wrist in attempt to catch himself.

Employee tugged on the air hose he was using and fell back catching himself on the hopper.

Employee was fueling haul truck. He was walking back to truck and slipped on ice. Conditions that week consisted of snow and ice.

outside conditions where slick, employee dismounted railcar and slipped on slippery stone.

snowman had formed in cooler . preasure washer was used to remove snowman.

MCC went to ground. Power was turned off and breaker was repaired. When MCC was renergized, the breaker blew and caused a flash burn to hand. New breaker was believed to be faulty.

Pumping water out of containment area. Ladder slipped, causing employee to fall on his wrist.

Removing air pad from bottom of cement silo. Silo had been emptied leaving a small amount of cement on the pad. While the pad was being removed a small amount fell onto his face irritating his eyes.

EE was ascending side slope of a limestone stockpile to get elevation of top of slope. He lost footing of lt foot, started to slide. Trying to catch himself his rt leg twisted & collapsed under his own weight. The rt femur was broken at a location of a previous break. According to EE, the bone was previously broken 4 times due to a degenerative bone disease.

Employee was carring powershot tubes to and from 308 bin. He was utilizing the stairs because the elevator was down for repairs. Weather conditons were not a factor and all ppe was worn. The employee complained of a strain/pull in his lower back.

Employee was retrieving a hot meal sample, when an air cannon discharged 30ft below the sample line. This forced some hot air and dust back into the line and out of the opening. The employee was wearing all ppe but the dust still managed to travel under his faceshield, which resulted in his eyes being scratched.

No injuries, dust collector cartridge replaced. This was an unplanned explosion in the coal mill. Mill was designed to absorb and release energy and respond in the correct manner. No injuries & little damage

preheater tower went positive through the riser duct, pushing small amount of dust into the air, which went underneath EE's faceshield and safety glasses. All ppe was present.

When lifting mill door into place the anchor point broke free causing a whiplash of lifting sling while the sling was in employee's grasp causing a deep laceration to his left hand.

Employee had 2nd and some 3rd degree burns to his underarm/back area. We believe that he backed up to the muffle oven to warm up when he caught his shirt on fire after being outside getting a slurry sample.

EE was lifting a 90lb steel brace for a conveyor and the brace slipped out of his hands causing his back to jerk, resulting in a sprain of the lower back.

EE's shift was having problems with #1 finish mill. EE went out to see what was wrong with the finish mill. He found that the tipping valve was stuck open coming from dust collector. When he pushed the valve arm down, the counter weight slid down the handle and hit his right hand cutting it.

He was installing scaffolding when a piece of material hit a tube causing it to rise up and hit his hand and fracture his left wrist.

Employee was helping a co-worker repair a broken port hole door on the 3rd floor of the tower riser when some hot dust flowed through the opening and got in his eyes. He had PPE but the dust went underneath and got in his eyes.

Employee was loading CKD dust from the dust tank to a haul truck using the pug mill and he slipped when loading the pug mill and fractured his right arm.

Employee was working on the 513 dust collector and fine dust fell on him from up above causing his throat to be irritated.

Employee pulled a groin tendon from falling in a hole while replacing grade bars in clinker cooler in a limited work space. It was dark and the floor had debris on it.

Miner was doing repair work and got clinker dust in his eyes while removing equipment parts.

WHILE BACKING UP THE HAUL TRUCK, EE HIT A RUT IN THE ROAD. EE GRABBED THE STEERING WHEEL, WHICH CAUSED HIS SHOULDER TO POP.

Employee was moving a 55-gal. drum and twisted his back in the process. As he twisted he felt a pain in his back. Injury is still bothering him, so medical attention was sought. Now an MRI is scheduled, became reportable on 8/10/2004.

EMPLOYEE WAS ASSISTING PLANT PERSONNEL IN MOVING A HEAVY ROTOR FROM PLANT PRODUCTION AREA TO A NEARBY STAGING AREA. A LOADER WAS BEING USED TO TRANSPORT ROTOR, AS LOADER WAS LOWERING LOAD THE RIGGING BROKE WHICH SECURED THE ROTOR. THE ROTOR THEN STRUCK A RAILROAD TIE WHICH IN TURN FLIPPED & CAME DOWN ON EMPLOYEE'S RT.FOOT.

EMPLOYEE WAS ASSISTING FELLOW EE LIFT A PIECE OF TUBE STEEL APPROX.6'LONG UP ONTO A PAIR OF SCATES USED FOR MOVING HEAVY LOADS, EE LOST HIS FOOTING/ BALANCE BUT RE-GAINED CONTROL TO FINISH THE LIFT BUT IN TURN PUT HIMSELF IN A SOMEWHAT TWISTING POSITION WHILE STILL MAINTAINING THE LOAD ALONG WITH THE FELLOW EE.

HOISTING DOOR UP TO A LANDING TO INSTALL DOOR IN UNIT. EE'S HAND GOT CAUGHT ON BACK SIDE OF DOOR. LEFT HAND FINGER BROKEN

EE HOOKED UP DISCHARGE HOSE ON TANKER TRUCK TO DUMP FUEL AND TWISTED WRONG AS HE CAME OUT FROM UNDER THE TRAILER.

EE SLIPPED ON LOOSE ROCKS WHILE REPLACING BOLTS IN A ROCK FEEDER AND FELL A COUPLE OF FEET. HIT HIS HEAD AND LEG ON FEEDER SUPPORTS.

Employee's coveralls caught fire wile EE was working to clear a blockage in the preheater tower. Hot materials ignited coveralls and caused burns to legs before EE could get them off.

EE WAS STANDING NEAR A CONTRACTOR WORKSITE OBSERVING THE WORK. A METAL SHAVING CAME INTO CONTACT WITH HIS LEFT EYE AND SCRATCHED HIS CORNEA. AN EYE DOCTOR REMOVED THE METAL AND PRESCRIBED AN ANTIBIOTIC.

EE STATED THAT A BAG SHE FILLED FELL TO THE BELT AND SHOT CEMENT OUT THE OPENING STRIKING HER IN THE EYE.

EMPLOYEE WAS DOING REPAIR WORK ON BELT WITH COWORKER. HE SAID HE TWISTED AROUND TO LAY DOWN PIECE OF PIPE AND STRAINED HIS BACK.

TWISTED BACK GETTING OFF SHOVEL ON 9-4-03. BACK STILL SORE 9-15-03. WENT TO SEE CHIROPRACTOR MULTIPLE TIMES FOR ADJUSTMENT DURING PAST WEEK.

EMPLOYEE STRAINED HIS BACK WORKING IN TUNNEL. HE BENT TO REMOVE GUARD AND TWISTED WRONG.

EMPLOYEE WAS PULLING A TANKER DOME LID OPEN AND PULLED/ STRAINED HIS LOWER BACK. RESTRICTIONS MADE THIS A LOST TIME ACCIDENT.

EE ALLEGES WHILE LOADING ROCK INTO A FEEDER HE DUMPED BUCKET AND SHIFTED INTO REVERSE, WHEN LOAD SHIFTED IT JARRED HIM, HURTING HIS NECK. FOLLOW UP TESTING CONFIRMED INJURY.

EE WAS MOVING FLANGES FROM A PALLET TO A STOREROOM SHELF. HE STRAINED HIS BACK BUT THOUGHT IT WOULD GET BETTER. IT HAS NOT. HE WENT TO OUR PLANT DR. AND WAS REFERRED TO A SPECIALIST. RESTRICTI ONS MADE THIS A LOST TIME.

EE WAS ASSISTING WITH WORK ON FK PUMP. HE WAS WORKING ON COUPLING WHEN FINGER GOT SMASHED IN COUPLING. TIP OF 2ND FINGER OF RIGHT HAND WAS CRUSHED.

WALKING ACROSS FLOOR WITH METAL GRATING, CAUGHT RIGHT FOOT, FELL, SPRAINED RIGHT ANKLE.

EE WAS APPARENTLY MOVING 1" THICK PLATE STEEL WHEN INJURY OCCURRED. HE STATED HE WAS MOVING A LARGE PIECE OF THIS STEEL WHEN HE STRAINED HIS LOWER BACK. PROBLEM IS STILL BOTHERING HIM - HE HAS BEEN REFERRED TO A SPECIALIST.

EE FELL THROUGH 6"OPENING IN GRATE WHILE PUTTINGUP A STATIC LINE. HE TWISTED HIS KNEE AS HE WENTDOWN AND SUFFERED TISSUE DAMAGE.

EMPLOYEE STATED HE DROPPED AIR CANNON ON HIS THUMB. XRAYS WERE NEGATIVE. EMPLOYEE SAID THUMB STILL BOTHERS HIM. DOCTOR REFERRED HIM TO SPECIALIST ON 7/31/02 SUSPECTING A FRACTURE. *NO FRACTURE -PLEASE MAKE NOTE OF THIS.

INJURED WAS USING BREAKER BAR TO BREAK UP BUILD UP IN A GYPSUM FEEDER. WHEN BLOCKAGE BROKE LOOSE, WORKED SMASHED HAND FINGER BETWEEN T BAR AND SHOOT.

FOREMAN WAS WATCHING WORKER HAMMER ON STEEL SHAFT. PIECE OF METAL STRUCK FOREMAN IN NECK, CAUSING A LACERATION. FOREMAN WAS BOTHERED BY NECK PAIN ON 5/20/02 & HAD DOCTOR CHECK IT. PIECE OF MET AL IS STILL IN NECK & NEEDS TO BE REMOVED SURGICALLY.

WHILE WORKING IN MILL HAMMER HEAD CAME OFF & STRUCK WORKER IN MOUTH, BREAKING TOOTH.

MINER WAS RETRIEVING A TOOL AND HIT HEAD ON I-BEAM. MINER FORGOT ABOUT I-BEAM AND WALKED INTO IT. PAIN WAS NOT A PROBLEM UNTIL 2-27-02. WORKER CAME TO WORK ON 2-27 SAYING PAIN WAS WORSE. HE WA S THEN SENT FOR TREATMENT.

GRINDING SURFACES FOR WELDING. FRAGMENTS GOT INTO EYES DESPITE WEARING PROPER SAFETY EQUIP.

EE WAS ASSEMBLING DUCTWORK WHEN SOME OTHER DUCT WORK ROLLED DOWN AND STRUCK HIM. THE EE SUFFERED MINOR BRUISNG ON HIS RIGHT SIDE OF BACK AND RETURNED TO WORK LATER SAME DAY. EE HAD NO FURTHER COMPLICATIONS.

EE WAS WALKING TO PUT AWAY TOOLS AND STEPPED IN A HOLE IN THE FLOOR AND FELL.

EE WAS CUTTING GALVANIZED GRATING AND INHALED FUMES OF ZINC OXIDE THROUGHOUT THE DAY. LATER THAT EVENING, THE EE BECAME ILL. HE WAS THEN TAKEN TO THE HOSPITAL E.R. AND TREATED FOR ZINC OXIDE POISONING.

2 MINERS WERE USING AIR HAMMER TO LOOSEN HARDENED CEMENT AND MILL BALLS FROM THE WALL OF THE BALL MILL. A LARGE PIECE BROKE LOOSE AND STRUCK THE VICTIM.

MINER WAS FULLING CLINKER CHUNKS OFF OF CLINKER BELT WHEN A LARGE CHUNK FELL OFF AND STRUCK HER FOOT.

WHILE MOVING PALLETS WITH PALLET JACK EMPLOYEE WAS PINNED BETWEEN SUPPORT PILLAR AND LOAD OF PALLETS. EMPLOYEE RETURNED TO DOCTOR ON 10-5-01 SORE CHEST AND WAS PUT ON RESTRICTIONS (WORKED REGU LAR JOB UNTIL 10-5-01).

MINER WAS INVESTIGATING PROBLEM AT GYPSUM FEEDER SHOOT AND TWISTED BACK WHILE LOOKING UP AT FEEDER.

MINER WAS WORKING IN BALL MILL W/O OTHER MINERS. A BALL WAS KNOCKED LOOSE WHILE HAMMERING. THE BALL STRUCK MINER IN THE EYE SOCKET.

MINER HIT HEAD ON OVERHEAD HAZARD WHILE CLIMBING INTO PICKUP BED.

MINER WAS CUTTING METAL WHEN IT SHIFTED. MINER CUT THROUGH GLOVE AND CUT HAND BETWEEN THUMB AND INDEX FINGER.

EE WAS WORKING ON WELDER AND CAUGHT TIPS OF LAST 2 FINGERS ON RIGHT HAND IN FAN BLADE CUTTING ONE AND FRACTURING OTHER.

WALKING OFF BOARD ONTO THE CLINKER COOLER FLOOR, STEPPED SHACKLES ON FLOOR OF CLINKER COOLER TWISTING RIGHT KNEE.

EE SLIPPED & FELL IN BED OF SERVICE TRUCK WHILE REFUELING EQUIPMENT.

EE STUBBED TOES CAUSING FRACTURE OF PINKY TOE-LEFT FOOT.

WHILE CLEANING EE SMASHED HAND BETWEEN ANGLE IRON AND CLEANING ROD. ON 1-8-01EE WAS REFERRED TO CLINIC FOR AN XRAY BECAUSE HAND WAS STILL SWOLLEN. THE XRAY SHOWED A BROKEN BONE.

MINER WAS PULLED INTO BELT WHILE ATTEMPTING TO KNOCK FROZEN CHUNKS OF MATERIAL FREE FROM RETURNIDLER.

TWO EE WERE TRYING TO OPEN THE HAMMER MILL DOOR BY REMOVING THE NUTS ONT HE BOLTS. THE WRENCH SLIPPED AND JUMPED BACK TWOARD THE EE, STRIKING HIM ON THE FACE.

EMPLOYEE WAS REMOVING 3/4 SHEETS OF PLYWOOD FROM DECKING. WHEN HE PULLED UP ON A PIECE, IT HUNG UP, BROKE LOOSE AND HIT THE ELBOW AGAINST THE BEAM SUPPORT.

EE WAS INSTALLING CONVEYOR PARTS, TWISTED, AND STRAINED THE BACK. EE RECENTLY UNDERWENT PHYSICAL THERAPY SESSION, INCIDENT NOW REPORTABLE.

EMPLOYEE WAS INSTALLING A BEAM, USING AN 80 LB HAMMER. FELT PAIN IN THE ARM/ SHOULDER AREA. RECENT SUBSEQUENT TREATMENTS NOW MAKES THIS INCIDENTREPORTABLE, AS OF 5/16/00.

WHILE CLIMBING THROUGH SCAFFOLD TUBING ON HAMMER MILL CRUSHER HIS FOOT SLIPPED ON LOOSE MATERIALON SCAFFOLD BOARDS.

EMPLOYEE WAS PULLING ON A COME-A-LONG WHEN HE FELT A PULL IN THE GROIN AREA. EMPLOYEE DID NOT REQUEST TO SEE A PHYSICIAN UNTIL 4-13 WHEN THE SYMPTOMS SEEMED TO WORSEN.

WHILE REMOVING CONCRETE FORMWORK THE FORMWORK SUDDENLY RELEASED UNDER PRESSURE FROM CROW BARS WHICH WERE BEING USED TO PRY THE FORMWORK OFF. THEFORM SPRANG OFF AND TRAPPED HIS HAND AGAINST THE SCAFFOLD PLATFORM POLE.

EMPLOYEE WAS PULLING A FUEL HOSE OUT OF A FUEL TRUCK AND SLIPPED, FALLING INTO A PIECE OF IRON. EMPLOYEE DID NOT REQUEST MEDICAL TREATMENT UNTIL 4-25-00.

WHILST TRAVELING IN CONSTRUCTION ELEVATOR EMPLOYEE WAS TAKEN TO THE 300 LEVEL ON ROUTE TO THE 230 FT LEVEL FOR MUD. AT THE 300 FT LEVEL HE WAS ENVELOPED IN STACK EMMISSION FUMES. HE LEFT FOR W ORK AT 12 PM AND BECAME ILL WITH A SORE THROAT. HE LATER DEVELOPED A SERIOUS MEDICAL CONDITION.

EE WAS HELPING ANOTHER TO MOVE A VICTAULIC GROOVER DRIVE MULE OFF A PLATFORM. THE EE'S FINGERS GOT PINCHED BETWEEN THE BACK OF THE MOTOR AND THE CONCRETE PAD WHEN THEY PULLED ON IT.

COLD AND ICY DAY. PULLING PALLET JACK WHILE MOVING BRICK SLIPPED AND FELL DOWN ON HIS KNEE.

EE WAS BOLTING IRON WITH ANOTHER. THE LOAD SHIFTED, CAUSING THE BEAM TO FALL AND SMASHED THE EMPLOYEE'S HAND BETWEEN THE IRON.

EE WAS STRIPPING 500 MCM WIRE, KEEPING AN EYE ON WORKERS ABOVE. THE KNIFE SLIPPED AND CUT THE FINGER.

EE FELL ON SNOW AND ICE WHILE PULLING NEW ELECTRICAL SUPPLY CABLE THROUGH CONDUIT. EE COMPLAINED OF PAIN ON 2-2-00 AND WAS PUT ON RESTRICTED DUTY UNTIL 2-7-00. RESTRICTED TO LIFTING 10#.

EE SLIPPED ON A LARGE ROCK UNDERNEATH THE SNOW STRAINING HIS LEFT KNEE.

EE HAD BEEN WORKING IN THE PRE-HEATER TOWER FOR APPROXIMATELY 2 HOURS. HE WAS IN THE PROCESS OF PUTTING CONDUIT IN THE TOWER. CO-WORKERS HAD SEEN HIM SITTING DOWN AND TRIED TALKING TO HIM, BUT HE DID NOT RESPOND. EE HAD DIED OF NATURAL CAUSES ACCORDING TO CORONER'S REPORT. THE REPORT ALSO STATED THAT THE EE DID NOT DIE FROM ELECTRICALEXPOSURE OR TRAUMA. NON-OCCUPATIONAL DEATH.

EE PICKED UP A CABLE, IT SLIPPED AND LANDED ON HER KNEE.

A WRENCH FELL FROM ABOVE AND STRUCK THE EE'S HAND.

SAID HE HURT HIS BACK MOVING STEEL PLATES WITH PALLET JACK. ALSO SAID HE WAS GOING TO PICK UP THE PALLET JACK TO PUT IT ON TO THE ELEVATOR WHEN PULLED HIS BACK. NO PLATES HAD BEEN NOVE OF MATE RIALS NOTIFICATION OF INJURY NOT MADE AS WORKERSTCOMPENSATION RECORABLE OR POSSIBLE RECOREABLE TIL 1-11-00 CO DISPECTES ACCIDENT.

WHILE TRYING TO UNPLUG A RAW MATERIAL FEED CHUTEUSING A JACKHAMMER THE JACKHAMMER SLIPPED OFF THE MATERIAL & ALLOW EE HAND TO BE CAUGHT BETW3EENMETAL OF THE CHUTE & THE TOOL. RESULTING IN SMAS HED FINGER & DR VISIT, PRESCRIPTIONS

EE WAS HELPING TO PULL CABLE THROUGH THE HEAD SECTION OF A BOOM ON A 4100 CRANE. THE END OF THE CABLE CAME THROUGH AND BECAUSE OF THE KINK IN THE BECKET, IT STUCK THE EE ON THE SAFETY GLASSES CAUSING A LACERATION ON THE CHEEK.

EE HAD BEEN IDENTFYING SOME EXISTING WIRING. HE TERMINATED ONE OF THE WIRES AND THE OTHER BRUSHED UP AGAINST THE EXPOSED BUS BAR. THIS CAUSED A FLASH WHICH BURNED THE EE.

EMPLOYEE WAS RIGGING IRON WHEN THE RIGGED LOAD CAME OFF OF THE GROUND. HE TURNED AROUND TO GET OUT OF THE WAY AND HIS LEFT FOOT CAUGHT ON A 4 X 4 THAT WAS USED TO CRIB UP THE OTHER PIECES OF I RON. INCIDENT CAUSED A STRAIN TO HIS LEFT KNEE.

EE WAS PERFORMING MAINTENANCE ON SHALE BIN HOPPER, USING CUTTING TORCH. FOREIGN OBJECT BLEW INTO EYE. EE WAS SENT TO EYE DR. AND FOREIGN OBJECT REMOVED. EYE DROPS GIVEN TO USE FOR ONE DAY.

EMPLOYEE WAS PLACING RODS BACK INTO RAW MILL. WHILE GUIDING THE RODS THROUGH THE MILL OPENING A FLAKE OF RUST BLEW INTO EYE. THE EMPLOYEE FLUSHED THE EYE AND WAS SEEN AND ATTENDED TO BY EYE DOCTOR, WHO REMOVED A PARTICLE OF DEBRIS. EYE DROPS WERE GIVEN.

EMPLOYEE WAS LAYING BRICK AND DROPPED A BRICK ONSMALL FINGER OF HIS RIGHT HAND.

AN ELECTRICIAN CAME IN CONTACT WITH A LIVE CIRCUIT WHILE PERFORMING NORMAL ANNUAL MAINTENANCE. THE EMPLOYEE WAS TIGHTNING TERMINALS ON THE BREAKER OF A 30AMP CIRCUIT AND HIS SCREWDRIVER CONTAC TED GROUND, RESULTING IN A FLASH BURN TO HIS HAND AND FOREARM.

EMPLOYEE IS A REPAIRMAN. WHILE REMOVING HEAD LINERS FROM RAW MILL, A COLD CHISEL BOUNCED BACK AFTER BEING STRUCK AND HIT EMPLOYEE IN FOREHEAD. EMPLOYEE WAS TAKEN TO HOSPITAL , TREATED/RELEASED HAD SEVERAL STITCHES AND RETURNED TO WORK ON THESAME DAY.

EE WAS TEARING OFF PAGE ON CALENDAR. THIS CALENDAR HAS METAL BINDING. WHILE TEARING OFF PAGE, HIS FINGER WAS CUT BY CONTACT WITH METAL PART. HE RECIEVED 2 STITCHES.

HIT PROTRUDING ROCK IN ROADWAY.

UNLOADING PORTABLE CONCRETE FORMS, FINGER THUMB GOT CAUGHT BETWEEN FORM AND STRUCTURAL SUPPORT BEAM. CUT ON THUMB CLOSE TO NAIL REQUIRED SIX STITCHES AND SEVERAL DAYS RESTRICTED DUTY.

DRIVER WAS DRIVING ACROSS QUARRY FLOOR GOING TO GET A LOAD OF ROCK WHEN HE CROSSED A SMALL LOW DITCH IN ROAD, HE MUST HAVE HIT HIS HEAD ON RIGHT SIDE OF CAB THEN THREW HIM TO LEFT SIDE WHERE H E HIT THE DOOR GLASS WITH HIS HEAD THEN LET THE TRUCK ROLL TO A STOP.

MILL DEPT UTILITY MAN WAS CLOSING A SLIDE GATE ON HOPPER FEEDER. WHILE USING PIPE WRENCH ON GEARED SHAFT, THE WRENCH SLIPPED OFF AND CAUGHG FINGER BETWEEN WRENCH HANDLE AND SIDE SKIRTING. CAUS ED LACERATION AND CONTUSION. REQUIRED STITCHES.

EMPLOYEE HAULING ROCK (35 TON TEREX) WAS BEING LOADED BY HITACHI EX1100 SHOVEL WHEN LARGE ROCK APPROX. 8' X 3' X 3' FELL FROM SHOVEL BUCKET TO BED OF TRUCK (HEIGHT ~2') CAUSING EMPLOYEE TO BE KNOCKED AROUND INSIDE CAB. EMPLOYEE SUFFERED CERVICAL STRAIN AND SPASM (WHIP LASH). REQUIRED X-RAYS AND PRESCRIPTION MEDICINE- RESTRICTED DUTY 5 DAYS.

WHILE EE WAS DIGGING OUT RAW MILL SUMP, HE CAME IN CONTACT WITH A SHARP EDGE OF A DISCHARGE PIPE. CONTACT RESULTED IN A CUT OF HIS WRIST. EE WAS BLEEDING AND TAKEN TO EE RM WHERE HE RECEIVED S TITCHES AND TETANUS SHOT.

EMPLOYEE WAS USING HAND HELD GRINDER. WHILE GRINDING A PIECE OF METAL, GRINDER KICKED BACK CAUSING GRINDER TO COME IN CONTACT WITH EMPLOYEE'S ARM, WHICH RESULTED IN LACERATION ANDREQUIRED STIT CHES AT HOSPITAL EMERGENCY ROOM.
