Looney Creek Taggart Mine: 569 MSHA citations Coal

A roof fall was found in #6 intake entry 1 brk. inby SS #847. Area was timbered and dangered off.

Received a CWP diagnosis on 4/25/14. Employee was termed on 7/18/12 and is no longer employed with the company. Accident date of 7/18/12 is to accommodate online report per MSHA (Denver) instructions.

A roof fall was discovered 7/10/2012 at approximately 10:30am by the mine foreman. The fall is located in the A Right Panel in the right return at survey spad #'s 669, 670, and 671. These locations are in the number 5, 6, and 7 entries.

@ Northwest Mains Belt Entry SS#1659. Crew were loading belt on trailer with a scoop when a belt dog slipped loose hitting worker on left hand.

Roof Fall located in # 4 entry at SS #6 measuring 20' wide, 30' long and 8'high No ventilation or equipment effected.

Employee was watching water line being pulled, when the water line broke striking the employee.

Injured was operating a rubber tired battery powered conveyance traveling to a work site and failed to maintain control, striking a large metal jack, a part of which fell and struck his ankle or foot.

Possible occupational illness, due to exposure to inspirable dust.

The individual was using a pry bar to align a head light on the Fletcher roof bolting machine. The bar slipped and caused him to strike his hand against the frame of the machine. This resulted in a fracture of his right ring finger.

The individual was backing the roof bolter out of #4 Left and his right hand was caught between the operator deck and the coal rib. He did not start missing work until 2-11-11.

The individual was roof bolting and had finished drilling the hole when the drill steel fell out of the hole hitting his left hand causing a fracture to the ring finger.

The EE was walking from the feeder to the shuttle car and stepped into a hole twisting his left knee.

While operating the continuous miner, a rock was thrown by the cutter striking the individual in the head above the right eye causing a contusion.

EE had installed a bottom roller on offside of the belt. While crossing back over the belt he placed his left hand on the structure to step off the belt. EE felt a sharp pain in his elbow when stepping off of the belt.

A roof fall was discovered in the #1 entry approximately 8,000 feet outby the section at survey station #875. The fall measured 16' wide x 35' long x 7' thick. There were no injuries or equipment involved.

The individual had hung curtain beside the roof bolting machine and when walking behind the machine he turned his ankle and felt his knee pop. He did not go to the doctor until 9/910 and was placed on restricted duty beginning 9/10/10.

The individual was unloading bottom belt rollers from the three wheeler, when he felt a pain in his back.

A roof fall was discovered in the No. 4 Entry of the 1 Main West set of Entries, approximately 30' outby survey station No. 879. The fall measured approximately 16 feet wide by 15 feet long by 8 feet high. There was no equipment involved. There were no injuries.

The employee was building a brattice and as he was driving a cap wedge over a block, his hammer hit the roof and a small piece of rock hit him in the right eye. He was wearing safety glasses. He went to the doctor and had the foreign object removed. This is a medical only report. There were no restricted or lost work days.

Fall was discovered at 9:56 pm on 2/21/10. The fall is approx 750' outby active section in the #2 entry 1 crosscut outby SS 867. The fall is approx 20' long X 20' wide X15' high. The fall did not affect ventilation travel didnt not cause any injuries and did not trap any equipment. The area has been timbered and dangered off and the fall will not be cleaned up.

EE was operating the continuous miner in the # 1 entry. EE was sitting on his feet and a small piece of rock fell off the rib knocking him off balance and into the miner striking his face on the frame of the miner.

The individual was on his knees putting cross overs on the miner cable. When he raised up, he hit his right ear on a rib angle cutting his right ear. The cut required stitches.

EE was scooping the front of the feeder when a roll of rock and coal rolled in the deck of the scoop striking the EE in the head and neck.

The individual was walking across the section and struck a rib hanger that wasa hanging down, causing a laceration to the right ear. The injury required stitches and is a medical only injury.

Individual was pulling slack up in mine cable and ran into a bolt hanging down from the top hitting the back of his neck on the bolt.

The injured and another miner were setting roof jacks along the haulway at SS #888. When the other worker took a jack from the jacks piled along the rib, another jack fell on the left foot of the injured.

A fall was discovered at 8:40 pm on 7-26-09 by foreman who started his shift at 6pm to preshift. The fall was in the #3 belt entry at SS No 881 of the No. 1 main west set of entries. The fall measured 20' wide by 25' long by 15' high. The fall will be cleaned up and the area supported.

The individual was operating the roof bolter in the #4 entry. As he was looking up at the roof while operating the roof bolting machine, a small piece of rock fell from the roof, striking the individual in the right eye.

The injured was hanging the electrial cable to the roof bolting machine. The machine operator engaged the cable reel and caused the cable to knock EE down to the mine floor.

The employee slipped and fell on the mine floor. He landed on his left hip resulting in a strain and bruise to his hip.

The injured was in the process of installing a roof bolt when a piece of draw rock fell and struck his left leg.

The injured was cleaning the dust box on the roof bolting machine when a piece of draw rock fell and struck him on his head and neck.

The continuous mining machine finished a left run out of the entry while pillaring. The injured was preparing to reposition the mobile roof supports when the mining machine accidently struck a piece of metal on the MRS. The metal shot off unexpectedly striking him on th right forearm. This resulted in a laceration which required sutures.

The injured was loading coal into a shuttle car with a continuous mining machine when a rock flipped off the machine boom and struck the individual on his left leg. This resulted in a fracture of his lower leg.

The employee was using a 3-wheeled rubber tired personnel carrier to pull up extra cable for the roof bolting machine. When he cut the rope loose from the 3-wheeled machine, he cut his left pointer finger. The laceration required sutures.

A roof fall was discovered in the 2 left panel off main east. The fall was in the NO. 2 return entry about 30' to the right of survey station NO. 236 and measured approximately 20' long by 15' wide by 6' high. The fall was approximately 1,400' ouby the working section and will not be cleaned up. There were no injuries or equipment involved.

EE was installing roof bolts using a dual head bolting machine. EE walked to the back of the machine to get a box of resign, when a piece of draw rock fell striking his right forearm. The injury resulted in a fracture and a laceration which required suitures.
