Love Branch Mine: 557 MSHA citations Coal

Fall discovered 12-02-2008 inby break 18 on 2A belt in main left return. The fall was 20 ft x 100 ft x 7>8 ft high. The area was already dangered, it was breakered off, and will not be rehabed. Both MSHA & OMSL were notified.

Had fall in face area, 4ft X 10ft X 7ft high; breakered area off, not to rehab area, moving section. MSHA & OMSL notified. No equipment involved, no people injured, no vent disturbed.

ROCK FALL; in main return on belt 2A, 20' X 70' X 5' high. No equipment involved, no ventilation affected. MSHA & OMSL notified, will rehab the area; clean up and install additional support.

Putting up strap, used hammer to adjust strap to drill next hole. Missed the strap and right wrist contacted strap, causing laceration. Employee was taken to the E.R. for treatment. Stitches were used to close the wound.

Truck dropped off the road & went in the ditch.

Fall was discovered at 2A belt, break #25 entry #2 about 11:00 A.M.. Fall was 20' X 70' X 6' high. Entrances to the fall were breakered off, and area will not be rehabilitated. Inspector investigated the fall.

06-12-08, @8:05 A.M. a fall was discovered on #2A belt at break 21, in #1 entry, in the return. The fall measured 20' X 140' X 6' high. All entrances to the fall area was breakered off. The area will not be rehabilitated. Inspector inspected the area.

Employee trimming plastic from cable hanger. Knife slipped, employee received injury to left forearm. Taken to E.R. for treatment, stitches used to close wound. RTW 5-2-08

Fall in the old Eastern Coal works area was discovered 11/20/07, #1 entry 4X-cuts inby portal. The fall measrued 20' X 60' X 8-10'. Area was breakered off, and MSHA and OMSL were notified, both agencies investigated. Return air had 50,000 CFM coming over the fall.

Fall discovered in #2 entry, #1 belt intake, break #6; Eastern's old works. Fall measured 20' x 60' x 6' - 7'. Area was breakered off, will be cleaned and additional support installed. Both MSHA and OMSL investigatied.

Operating SC, went under RB cable; flat rock on top of SC was pulled by RB cable, sliding under canopy into lap of employee. Taken to E.R. for treatment. RTW 10-17-07. 11-5-07 returned to doctor due to pain; removed from work for 2 weeks.

At end of track on section, man-trip slid on wet track (top sweating), employee holding onto bar behind seat, man-trip bumped flat car causing box with SCSRs to slide. SCSR box struck hand causing injury.

Fall on section 38' x 70' x 4' high. MSHA - OMSL notified. All entrances breakered off. Section not to be rehabed.

EE discovered a fall in the return at 3rd right panel, 1 x inby spad #2797. The fall measured 20' x 10'x 5' high, this area is in old works, MSHA inspector inspected the area 6/6/07 All entrances to the fall area had been breakered off, this area will not be rehabilitated.

Employee was replacing right cutter motor guard, the guard fell onto and cut the right ring finger. He was taken to the e.r. for treatment. He had stitches inserted to close the wound, and returned to work.

Employee stepped across tounge of mobile mine office, bumped head on air conditioning unit protruding from wall causing injury. He was taken to the E.R. for treatment and stitches were used to close the wound.

Parked rail truck, pulled in-bar fell between tracks. Lifted bar to move to side. dropped bar onto right foot wt. approx. 100-150 lb. causing injury.

Employee was helping two others hang miner cable. Area was muddy; while lifting and positioning it on the hangers, he developed pain in his lower back. He ceased work to let the pain subside but it increased. Employee was taken to the hospital for treatment. X-ray was negative; diagnosed with low back muscle strain.

Employee moved SC anchor, stepped across SC cable, bumped knee on side of car. Worked until 11:30 P.M., told supervisor of injury. Went to E.R. 5-19-06, released "if able to work", 5-22-06 saw specialist, taken off work with torn ligaments & fluid on knee.

EE injured his left thigh when he came in contact with the head of a Joy 1415CM. EE was tramming the miner, and got too close. The head struck him on the leg.

Riding in man-trip holding onto cage. Trip stopped finger struck by loose battery cover causing injury. Went to ER at end of shift, stitches inserted as percaution. Return to work 3/15/06.

EE was trimming roof at a cross-cut. A piece of draw rock fell onto his right ankle. He was taken to ER for treatment, X-ray revealed a fracture to the ankle.

12:45 am on the 001 section a fall measuring 20'x24'x3 1/2 to 4 inch thick occurred in the intake. fall was reported immediately. area has been cleaned up, and additional support added.

Member was changing filter on 4m when using a cheater bar on a wrench applied force breaking wrench causing him to mash his hands w/cheater bar & striking lft middle finger on c/m frame. Cut end of finger about 1/4" from tip of finger (received sutures.)

Front wheel of rubber tired personel carrier struck coal rib causing the steering wheel to spin rapidly. The assist steering knob struck injureds left arm causing a fracture to the radius bone.

AN UNPLANNED ROOF FALL OCCURRED JUST OUTBY 002 SECTION IN THE #7 ENTRY STARTING AT SPAD #2212 CONTINUING INBY TO SPAD #2226. FALL MEASURED APPROX 4.5 FT HIGH TO AN UNDETERMINED HEIGHT, 14 FT WIDE AND 230 FT LONG.

MEMBER WAS ADJUSTING FORK ON A FORKLIFT WHEN IT CAME OFF AND FELL STRIKING HIS RIGHT FOOT.

This is being reported in protest due to member did not report injury. First reference to an injury was 3/04/05/ Member claims to have hurt his back handling miner cable while working as a cont miner operator.

EE WAS COUPLING A ROCK DUSTER TO A 12 TON LOCOMOTIVE. AS HE WAS INSERTING PIN INTO PLACE, HIS CO-WORKER BACKED THE LOCOMOTIVE, MOVING PIN, FOWLING EE'S FINGER AGAINST THE LOCOMOTIVE FRAME (LEFT INDEX FINGER) LACERATING THE FINGER TO THE POINT OF THE AMPUTATION OF THE TIP FLESHY END OF THE FINGER.

EMPLOYEE WAS HAULING SUPPLIES WITH A RAIL LOCOMOTIVE TO THE #2 SECTION. CAR DERAILED WHILE HE WAS PUTTING THE SUPPLY CAR BACK ON THE TRACK. THE LIFTING JACK FAILED, CAR FELL, STRIKING HIM ON THE LOWER LEFT ARM.

Employee stated he was shoveling coal with a #4 aluminum shovel from the location of the 'land mine' under the belt guard at #6 belt head. He shoveled a load of coal and mud pulling the shovel load out to place it on the belt, twisted around, and as he lifted the shovel, he experienced a pulling sensation in his back.

There occurred a roof fall at this mine located at #3 entry inby spad #362 about 2160ft from the working face. It fell on highline cable in the intake. The size of the fall was 20ft L x 20ft W x 6ft H. Conventional 4ft bolts were used and strata was laminate. Ventilation was not affected and there will be no rehab.

Injured and his coworker were laying block as they were building a stopping. It was stated that 3 to 4 blocks fell and they tried to catch them to keep them from falling. Both of them recieved lacerations that required stitches.

Employee was in #7 entry working on the miner cable by himself. He said as he was pulling on the cable trying to get it out of the conduit, he twisted his left knee.

Employee was running the Roof Bolter in the 5R section crosscut. He noticed a crack in the roof running across the entry. He prepared to support the crack by laying a strap across the ATRS. When he did, the strap slid off the ATRS and struck him in the top lip causing a small laceration. He sought medical attention and recieved 2 stitches to the upper lip.

EE WAS MOVING BOLTS TO THE SIDE TO GAIN ACCESS TO THE OIL FILL LID. THE BOLTER OPERATOR WAS TRAMMING THE BOLTER MACHINE AND DID NOT SEE EE. THE MACHINE RAN OVER HIS FOOT (LEFT).

THE EE WAS SUPPLYING HIS BOLTER WITH ROOF BOLT PLATES. THEY HAPPENED TO BE WET AND SLICK. HE HAD A BUNDLE OF PLATES IN HIS HAND WHEN THEY BEGAN TO SLIP THROUGH HIS FINGERS. HE TRIED TO CATCH THEM AND CUT HIS FINGER ON THE EDGE OF ONE OF THE PLATES. HE HAD GLOVES ON AT THE TIME OF THE ACCIDENT. HE RECEIVED 7 STITCHES TO HIS LEFT MIDDLE FINGER.
