Employee was positioning a set of metal cribs under monorails, to load on a flatbed truck, when he felt a burning in his right knee. He was given a knee brace.

Operating a Hitachi 1700 Rock Truck traveling up an incline to the dump. As the truck approached the top section of the ramp, a large rock shifted causing the truck's front wheels to come off the ground and then back down, jarring the member. He returned to work, then on 4/1/2011 he was taken off work until 4/11/2011.

Going down hill on New Road. Ran over something, tore out plug in bottom of fuel tank & lost all the fuel out. Motor died out, lost brakes. Truck was checked at the scene. Was driver's fault.

EE was dismounting a DML 45 Rock Drill when he slipped and fell. He was taken off work on 10/15/10 after an MRI revealed he had a bulging disc and a sprained left knee.
