An employee was removing a lever hoist used to align a shaft during a bearing replacement. During the lever hoist removal, an employee's nose was lacerated when struck by the lever hoist. The initial X-ray results were negative for any fractures or broken bones. A fracture in the nose bone was revealed during a follow-up CT/MRI scan on 3/3/2026.

On 1/25/25 at approx. 8:30am a drill operator was injured when a hook from the drill winch cable fell from the top of the drill mast. The hook struck them over the left eye causing a laceration over their left eye. EE was taken to the ER, receiving multiple stitches. The operator had a follow-up appointment on 1/27/25 was released back to work 1/28/25 without Restrictions.

Operator was climbing down from machine and was using 3 point contact and missed the last step and fell to the ground and fractured their right ankle.

Employee was attempting to remove rubber from a blown tire that was wedged between the rim of the truck. As he attempted to cut the rubber with his pocket knife, the knife slipped cutting two fingers on his left hand.
EE's were lifting the base off the to of machine at the scales. After the base was lifted, the swivel on the clam shell moved causing the base to travel back 12", catching the EE's left leg between the base and electrical bax attached to the midrail. This caused a lacertion to his leg.

Employee was installing a chute on the load out bin, utlizing a come a long. Come a long slipped & the chute twisted, which caused the ee's hand to be caught between the chute & the come along. Employees left index finger was severly cut and smashed. Injury resulted in stitches to the finger. Employee was wearing personal protective equipment, when the accident occured.
He mechanic was working on a brake cylinder which was mounted on truck. Started to tighten bolts and and bolts stripped from studs causing chamber to separate.
After going under a conveyor frame, the employee rose up into a structural I beam resulting in a broken nose, cut upper lip having 5 stitches, chipped front tooth and cut on forehead.

EE HAD NO PAIN IN WRIST/HAND ON THE FOLLOWING DAY (FRIDAY), HE NOTICED PAIN & TIGHTNESS ON SATURDAY 3/20/04.
EMPLOYEE WAS STRIPPING THE WIRES DOWN ON THE SMA SCREEN MOTOR, WHEN THE KNIFE ROLLED OFF THE WIRE AND STUCK INTO HIS LEFT HAND CAUSING A 1 1/2" LONG LACERATION TO THE INSIDE PALM OF HIS LEFT H AND REQUIRING FOUR STITCHES.
