EE WAS 15FT BACK FROM FACE HELPING PUT A LEG ON 4 MACHINE HE WAS WORKING ON HIS KNEES AND BENT OVER. A SMALL ROCK FELL FROM THE BACK 8FT DROP AND HIT HIM IN THE MIDDLE OF THE BACK.

EE WAS DRILLING BEING TRAINED-HE WAS PULLING OUT OF A HOLE IN THE BACK, MACHINE WENT OVER CENTER AND FELL. HE HAD HIS HAND BETWEEN FORWARD PART OF DRILL END LEG AND WAS PINCHED WHEN THE MACHIN E JACK KNIFED.
