WORKING IN A GLACIAL HIGH MOISTURE MATERIAL, WE HAVE A PROBLEM WITH HIGHWALLS SLUFFING OFF OCCASIONALLY DURING RAIN SEASONS, WHICH IS WHAT OCCURRED THEDAY OF THE ACCIDENT. THE SUPREVISOR INSPE CTS THE HIGHWALL DAILY AND THERE WAS NO INDICATION OF A PROBLEM THAT DAY. THE HIGHWALL SLID IN ON RIGHT SIDE OF TRUCK WAITING TO BE LOADED WITH COAL. EE WAS WELDING INSIDE COMPARTMENT OF SWING GRINDER. ANOTHER EE WAS WELDING ON TOP OF SWING GRINDER. HIS SPARKS STARTED A SMALL GRASS & OIL FIRE BENETH THE GRINDER. THE FIRE INCREASED RAPIDLY. DAN CRAWLED OUT OF THE GRINDER THROUGH THE FIRE & WAS BURNED.
