THE EMPLOYEE WAS USING A TOOL WITH A HOOK ARRANGEMENT ON THE END IN ATTEMPT TO LIFT, ROTATE AND/OR FREE A STUCK ROCK. THIS HAND WAS CAUGHT BETWEEN THE TOOL AND NEARBY STRUCTURE. HIS HAND WAS C UT AND REQUIRED MEDICAL TREATMENT.

EMPLOYEE WAS DRIVING ADUMPTRUCK LOADED WITH SILICA FROM THE QARRY TO THE CRUSHER. AS HE WAS CROSSING RAILROAD TRACKS HE WAS STRUCK BY A U.P. TRAIN ON THE DRIVER'S SIDE OF THE DUMPTRUCK. THIS CAUSED THE TRUCK TO BE PUSHED A SHORT DISTANCE AND A DERAILMENT OF SWITCH ENGINES. TRAIN CREW PULLED EMPLOYEE FROM WREACKAGE.

EMPLOYEE WAS ENTERING THE SHOP AT MAIDEN ROCK MINE.EMPLOYEE SLIPPED AND FELL ON THE ICE IN FRONT OF SHOP DOOR.THIS ARCH RECEIVES VERY LITTLE SUN DURING THE WINTER AND EMPLOYEES HAVE BEEN CAUTI ONED TO KEEP AREA SANDED.IF THE AREA HAD BEEN SANDED THE ACCIDENT WOULD HAVE BEEN AVOIDED.EMPLOYEE SAW DR AT A LATER DATE.
