Dixie Quarries Inc · Fall from scaffolds, walkways, platforms

EE took off fall protection. Ladder was approx. 20' from the edge of quarry. Moved the ladder but would not lay flat. When the ladder moved, it caught EE's pants and pulled EE forward toward the quarry hole. EE said did not trip or fall, the ladder bumped the side of the quarry and grabbed the back pants pocket. When EE's pants ripped, EE fell into the water in the quarry hole.
