EE was unblocking the cage crusher with a chipping gun. Broken rock flew out of the top of the crusher and hit EE directly under their safety glasses and into their left eye.

Employee slipped on ice at the lime plant and hurt EE's right knee as a result.

EE stepped off bottom rung of ladder; EE's pocket caught a pry bar which caused EE to fall. EE tried to stop fall with right arm while falling to ground. EE did not seek medical help or miss any time until recently. EE went to the doctor in July of 2019 and on 9/4/19 received an operation to repair a tear in EE's shoulder. Since the operation EE has been off work.
Employee was in the shop on knees working on the tracks of a track loader when employee started to experience pain on inside of left knee. Note: Employee did not seek any treatment until September 25th. Employee was hoping it just needed time to heal.
Employee was cleaning buildup at the dryer drum chute with an iron bar. The bar hit against a hard surface weld and ee heard a pop in arm.

Employee was walking between cement pillars and tripped and fell. Employee put employee's arms out when falling and hurt employee's left hand/wrist.
Employee crawled under a handrail to apply grease on a piece of machinery and experienced a sharp pain in lower back.
EE was holding onto an 80 lb plate and another EE had another 80 lb plate. The other EE's plate slipped so the EE put hand out to prevent the plate from falling on EE's foot. As a result, the EE hurt right elbow up to the shoulder: muscle/tendon.
Employee was doing a lot of jobs requiring wrenching which caused both hands to swell to a point of not being able to use them.
Employee was grinding and grinder kicked back and cut employee's knuckle.

Employee was cleaning up around the H-mill and hit the top of EE's head on the feeder
EE was welding and moving workplace when EE hit the welding lead which fell on to the table with the rod in stinger. (At this point EE had shield raised.) The EE went to pickup the welder which was arcing which caused burn to both eyes. EE also got a piece of dirt in the left eye. (Probably from rubbing it.)

Employee was carrying an elbow and slipped on the ice and his leg slipped out on him.

Employee was shoveling dirt at back bins when a sharp pain flew in his lower back.
Employee was welding and burned his upper right and left arms.

EE was installing a brake chamber on a truck while laying on a creeper. After crawling out and standing up he felt something in his eye.
Employee experienced possible signs of a heart attack. Employee was sent to hospital. The hospital determined that the employee had an irregular heartbeat which was treated. Employee was told he could return to work the next day.
Employee was using a utility knife to cut a mud flap when it caught on metal and he cut his left hand.

Employee was hand shoveling material at the pulverizing plant when he strained his right shoulder.

EE set up a ladder to check a pipe. The ladder slipped and kicked out at the bottom. The EE fell down with the ladder, approx 15 feet. The EE sustained lacerations to his right arm and lower left leg.
EE was cutting metal with a torch. A spark from the torch went into his left ear. The EE sustained a burn to his left ear, which became infected.
EE was driving a shaft out of a B-mill roll (rocker-head) with a sledge hammer and punch. He missed the punch and smashed his finger with the sledge hammer against the rocker head.
Employee was installing ploughs in the 6000 mill and pulled his back muscle.
Employee was lifting a bucket of tools from the bed of his pick up truck and pain flew into his elbow.

Employee bent over to pick up an elbow and strained lower back.
Miner was getting out of the skidster; he had to crawl on the bucket & step down. His foot caught on edge of bucket and he fell to the ground catching himself with his left hand.

EE WAS USING TOILET IN MECKLEY'S MAINTENANCE SHOP. GETTING UP FROM SEATED POSITION, HIS LEFT KNEE MADE POPPING SOUND, CAME OUT OF JOINT, CAUSING A GREAT DEAL OF PAIN. ONE-HALF HOUR LATER IT WE NT BACK INTO PLACE AGAIN. THIS EPISODE RESULTED IN TORN CARTILAGE IN THE LEFT KNEE.

EMP WAS WORKING TO RETRIEVE A HEAVY PIECE OF STEEL THAT HAD FALLEN ONTO CONVEYOR. IN PROCESS HE ATTEMPTED TO SET THE STEEL VERTICALLY ON A PLATFORM. WHEN HE DID THIS, THE STEEL FELL OVER PINCH ING HIS LEFT INDEX FINGER BETWEEN STEEL AND PLATFORM. THE WEIGHT OF THE STEEL FALLING CRUSHED THE TIP OF HIS FINGER. DR SET BONE ON TIP OF FINGER THAT WAS FRACTURED AND STITCHED SPLIT FINGER.
EMPLOYEE WAS INSIDE A CONFINED SPACE UNDER THE CATWALK, NEXT TO A SCREEN, TRYING TO UNBLOCK A CHUTE BY SHOVELING THE MATERIAL OVERTOP AN I-BEAM. IN THE PROCESS, HE TWISTED HIS BACK. HE THOUGHT IT WOULD GET BETTER ON ITS OWN, USED ICE ON IT &KEPT ON WORKING. FINALLY THE PAIN BECAME VERY SEVERE AND RADIATED DOWN HIS RIGHT LEG, MAKING IT IMPOSSIBLE TO WORK. DIAGNOSIS: HERNIATED DISK.
