Blowing slag with an air nibbler when a piece of metal enter left eye

Driver was opening lunch box 2 hours into the shift when front wheel of truck struck beam damaging rim and tire. He signed form stating carelessness was the cause of accident and was sent home for three shifts. This was on 6/9 he made no mention of being injured. On 6/11 he called and stated he needed medical information for filing workers compensation claim.
