EMPLOYEE WAS CUTTING A PIECE OF D-CORD. HE CUT TOWARDS HIMSELF AND SOMEHOW INADVERTENTLY SWUNG THE KNIFE UPWARD & OUTWARD THEREBY DRIVING THE BLADE INTO HIS LEFT FOREARM AND INFLICTING A CLEAN CUT IN HIS UPPER FOREARM. HE WAS IMMEDIATELY TAKEN TO THE HOSPITAL WHERE HE RECEIVED TREATMENT.

EMPLOYEE WAS HANDING PICKING 5 TO 14 ROCKS. HE PIKCED ONE LYING BELOW A ROCK APPROX. 16 IN DIAMETER. THE 16 ROCK SLIDE 2 1/2 FT DOWN THE BANK AND STRUCK THE EMPLOYEE S RIGHT FOOT HE SUFFERED A CONTUSION AND FRACTURE OF THE LARGE TOE.
