The EE was improperly loading steel into a drill. They laid the boom down & were adding steel directly into the carousel. One end of a steel fell out while handling it. This caused the other end to fall out & smash their thumb between another steel. Upon seeing a doctor, it was diagnosed the the EE ruptured a tendon in their right thumb.

Employee poked self in the eye with ear-piece of safety glasses on 08/23/2021 while trying to put them back on, and requested medical treatment on 08/25/2021. Was treated with an antibiotic eye drop and released straight back to work. No restrictions.

While removing a guard from C1 conveyor the guard struck the miner on the left forearm causing bruising and swelling. Miner reported the incident but stated that they felt that they would be fine applying ice to the arm and did not want medical attention at that time. On 6/18/19 the miner reported to work and the swelling was worse so they ere taken to the Hospital ER.

EE was climbing stairs on the Allis screen catwalk when he felt and heard a pop in his right knee. He stated that he was carrying a bucket with 12 bolts within (approx. 15 Ilbs) in his right hand and two wrenches in his left hand while he was climbing the stairs.

Injured party was attempting to clean aggregate buildup from side walls. of bin: stepped into bin, and onto buildup: material broke free causing the injured to slide to the bottom of bin. Remainder of material followed, burying injured to mid thigh area.

Installing a steel liner in the chute of the simplicity screen, the liner had a few small areas of steel slag along the edge. The employee tried to position the liner into place but was unsuccessfull. He then tried to position it by stepping on the suspended liner but in doing this caused the liner to shift, pinching his right leg between the liner and chute. Saw doctor 12/22/06.

Employee had completed taking off outer lid of a 5 gal bucket of oil. The employee pulled up on the bucket spout with his right hand when the pull tab on the spout broke causing his right hand holding a box knife to cut his inner left arm.

The employee was moving a manlift into the pit to add a section of pipe to water pump. While moving the lift down the haul road on the east side of pit, the employee ran over pieces of shot rock that were approximately 8 to 10 inches in size, causing the manlift to rock back and forth resulting in the employee falling forward and striking his hand.

A FIELD CONVEYOR HAD STOPPED BECAUSE A ROCK GOT JAMMED IN THE TAIL PULLEY CAUSING THE CONVEYOR TO STOP. AFTER THE ROCK WAS CLEARED FROM THE TAIL PULLEY, EE NEEDED TO SHOVEL OFF CONVEYOR TO GET IT TO ROLL. WHILE SHOVELING ROCK OFF THE CONVEYOR, THE BELT STARTED TO ROLL BACK CAUSING HIM TO GET PUSHED TO ROCK BOX.

THE CABLE HAD TENSION ON IT WITH NO WAY TO RELEASE THE TENSION. THE EMPLOYEE POSITIONED HIMSELF TO SAFELY CUT THE HOOK. WHEN THE HOOK WAS CUT A PIECE FLEW BACK STRIKING HIS LEFT KNEE CAUSING A MINOR CONTUSION.
