Employee twisted his back while helping load a shot at the quarry. The impairment is unknown.

Employees were standing on a loader bucket trying to free a conveyor head pully. The loader operator in error dumped the men off pinning one between the bucket and the conveyor. The injured employee was transported to the hospital and found to have no broken bones or internal injuries. Contributing factors were the cold wet weather and disregard of company policy.

WHILE INSPECTING A GEAR REPAIR JOB DONE AM 12-06-06 ON A ROLL CRUSHER AN UNKNOWN OBJECT CAUGHT AND CRUSHED EMPLOYEES MIDDLE FINGER.

WORKING AT THE SHOP WITH COWORKERS AND THE HEAD CAME OFF OF A SLEDGE HAMMER AND HIT HIM IN HIS HEAD.
THE EMPLOYEE WAS CHANGING A BELT, AND ANOTHER EMPLOYEE ACCIDENTLY HIT WRONG BUTTON AND TURNED THE ENGINE ON. THE BELT STARTED TURNING AND IT BROKE THE SKIN AND THE TIP OF THE MIDDLE FINGER ON THE LEFT HAND.

EE WAS HOLDING STEADY A GEAR THAT WAS SETTING ON A LEDGE PRIOR TO INSTALLATION. THE GEAR SLIPPED OFF OF LEDGE TURNING OVER AND LANDING ON EE'S FOOT. NO BONES WERE BROKEN BUT UNSURE ABOUT OTHER INJURY TO HIS FOOT.

INSTEAD OF CHANGING GEARS ON THETR ACTOR TO GO FORWARD, HE PUT IT IN ANOTHER REVERSE GEAR. UPON LETTING OU THE CLUTCH, THETRACOTR BACKED UNDER THE CRUSHER PINNING HIM BETWEEN THE CRUSHER FRAME AND THE STEERING WHEEL ON THE TRA
