EE WAS STANDING OUTSIDE BY CRUSHER, A ROCK FELL OUT OF THE CRUSHER AND HIT HIS RIGHT HAND AND BRUISED THE RIGHT HAND.
SUB-CONTRACTOR WAS MOVING HIS LIFT AND HIT OVERHEAD LIGHT BULB CAUSING IT TO BREAK AND FALL. THEGLASS CUT HIS LEFT SHOULDER.

HE WAS TIGHTENING A BOLT ON THE CRUSHER.THE WRENCH SLIPPED OFF THE BOLT AND HIS HAND HIT THE CRUSHER FRAME.BROKE HIS RIGHT HAND.
BELTS HAD COME OFF CRUSHER DRIVE PULLEY. EMPLOYEE TOLD FOREMAN FOREMAN INSTRUCTED EMPLOYEE TOSHUT DOWN & LOCK OUT CRUSHER MOTOR THEN REMOVE GUAARDS TO REPLACE BELTS, EMPLOYEE SHUT DOWN CRUSH ER BUT INSTEAD OF REMOVING GUARD HE STUCK HIS HAND THRU THE GUARD WHILE PULLEY WAS STILL ROTATING.
