Employee was putting drive belts on the crusher when employee's right foot got caught between the rotor and sidewall of the crusher. Employee was wearing composite toe boots when incident occurred. Toe/foot was broke in an open fracture at the bridge of the foot. Dr. pushed bone down and sewed up with 6 stitches.

Clear day. Employee was attempting to climb off loader to stop feeder for VSI Crusher. Employee slipped on step at rear of loader and fell injuring right knee.
