Injured worker was installing a track on a bulldozer. As EE picked up the back of track, the chain that is used to pick up the track slipped off the track and struck the worker in the mouth. Worker lost 2-3 teeth. Fracture involving left paramedian anterior maxilla.
Employee stated that EE was loading a scrap top roller into back of a pickup when EE felt a pop in EE's back.
Employee was replacing the grid in the screen deck. As EE prepped to weld, EE received a shock rendering EE unconscious and casing a small burn on the back of EE's right thigh. EE was sitting on the screen deck when the shock occurred.
Employee was helping install a section of pipe above a refuse vibrator. As EE was positioning the pipe EE's hand was caught between the pipe and a feed box mashing EE's finger, resulting in a non-displaced fracture.

Employee was walking down 54 refuse steps to platform and stepped on lump of coal size of a golf ball. This resulted in a sprained ankle.
Employee was working on the catwalk of a belt truss that was on the ground. The employee was handling the hoops for the belt covering when the truss turned over causing the employee to land on the ground and fracture their tibia bone in right leg.
Employee was washing floor and walked into DP cell which was 5'6" from floor and protruding 18" from side of sump. Employee has continued to work. EE started physical therapy on 6/17/2024.
When employee lifted the 5 gallon can of diesel out of truck bed, EE felt like EE pulled something in the right shoulder. EE continues to work and started physical therapy on 2/7/2024.
An ignition event took place at the top of the Big Clean Coal Storage Silo. Personnel in the area witnessed a large fireball rising from the top of the silo. The foreman had just completed the on-shift inspection of the silo area and was descending down the belt way when the event took place. There were no injuries reported during this event.

Employee was holding panel to be bolted on. Thought it was supported enough to stay in place. Took left hand off panel and it dropped onto finger on EE's right hand. This resulted in a fracture and 4 stitches.

The technician was removing a broken hand rail from the right side of a 777D. EE had the handrail off and turned to go down the ladder to hook to the new handrail to install. EE unhooked the fall restraint to come down the ladder. EE's left foot slipped due to mud and rain. EE did have three points of contact but the slip pulled EE and EE fell to the ground on their back.
Received a hearing loss diagnosis for employee on 9/27/2022.
Employee was walking from the 3rd floor to the 2nd floor. As employee placed foot on the 3rd step from the bottom, the step broke loose from the right side looking down causing employee to lose balance and fall to the concrete floor. Employee landed on right knee and arm.

Received a hearing loss diagnosis for employee on 8/3/2022.
Employee exited elevator and slipped and hit on EE's right shoulder. EE continues to work and started physical therapy on 7/14/2022.
Employee was standing a 24' ladder up to change a light on a pole. They felt their shoulder pop while lifting the ladder up to lean it against the pole. They continues to work and started physical therapy on 7/6/22.

Employee had removed an impeller from a pump and was rolling it up out of the housing and EE received a laceration to the right pinky finger requiring stitches.

A non-injury ignition/explosion occurred at an abandoned/sealed underground mine MSHA ID 44-07306. The explosion in the abandoned mine is believed to have occurred during a severe thunderstorm.

Employee was changing out a bag of floc. Some of the floc spilled on the floor and employee stepped on it causing EE to slip and fall. This resulted in a sprain to EE's right knee. Employee stated physical therapy on 8/3/20.

Beam came lose, swung out and struck head
EE was washing between two I-beams adjacent to the hose storage near the primary cyclones on the 4th floor of the plant. When EE finished, EE turned to store the hose, but EE's boot got caught between the beams. This caused EE to twist EE's knee and start to stumble. In an attempt to catch self EE states EE strained EE's lower back also.

Employee was walking inside thickener when he stepped in trough and tried to catch himself. He struck his right wrist on a piece of metal resulting in 15 staples.
Employee was standing on an insulating mat that was on a pallet helping trouble shoot issue. When he stepped off the pallet he stepped down onto a #3 stone turned his right ankle and fell resulting in sprain.

Employee was chipping slag off 4" pipe that has just been welded when a small particle of slag went around his safety glasses and welding hood and into his left eye. The revision is being generated pursuant to citation #8209468 issued on 1/20/15 & management disagrees that a violation of the standard cited exists
Received a hearing loss diagnosis for EE on 8/5/2014. Employee last worked on 8/31/09.
Employee was washing floor when the water hose got hung on chute of vibrator. He pulled the hose and felt a sharp pain in his left knee. Employee continued to work up until a doctor took him off work on 7/10/15.

Employee was removing box of soap from metal cabinet. Right hand came in contact with door frame cutting his right index finger resulting in stitches.
Employee was walking down the steps from the 6th floor to the 5th when he felt his right knee pop.

Employee was injured while assisting another employee cut a piece of 8" steel. The weight shifted once the steel was cut allowing it to drop approximately 2-3" striking his left hand.
Employee was cutting old pipe down. His hand was holding the flanges when the pipe rolled and cut his left hand between his thumb and fore finger resulting in 2 stitches.
Employee has been medically diagnosed with partial peroneal nerve dysfunction, related to prolonged periods of kneeling with knee pads as part of his normal daily activities as diagnosed by Dr. Kotay, MD per his 5/24/12 visit. We were notified of the diagnosis on 5/31/12.

Employee was removing protection screens and channel over sump while chipping rust off channel. He was punctured by a piece of screen that was extended above the channel he was preparing to remove. The puncture required 2 stitches.
Employee has filed for and settled a claim for job-related hearing loss .Claim was reported as of 6/11/2011.
Employee slipped on ice in parking lot going to the supply truck.

A water hose was wrapped around a valve and the employee was in the process of removing it, when the valve fitting blew off, striking and knocking the employee backwards. He fell down to the concrete, landing on his tool pouch and injured his left leg.

Employee was grinding and welding on top of discharge gate, a few days later he realized he had a small piece of metal in his eye. Eye presciption medicine was administered.
Employee was moving conduit when it broke and cut his hand.

Employee had checked the stacker belt and was walking around the breaker building and took a short cut between the power center box and the breaker building. He had to stoop to get under an airline. When he rose he struck his head against a heater.
Employee alleged he hurt his back while operating a d-10 dozer on the raw coal stockpile on 10-12-06. Injured did not reporat this accident until 10-20-06.
Employee was removing bolts in box bushing. Broke bolt. Striking his wrist against pump housing. Went to doctor 5/2/06. Returned to work on 5/3/06.

Injured was replacing the bottom deck discharge lip on a raw coal vibrator. As he stepped down from the vibrator to the floor he felt a tingle in his right knee.

INJURED WAS SITTING ON THE CATWALK AT THE HEAD END OF THE #54 REFUSE BELT CUTTING BOLTS OUT OF THE SNUB ROLLER. HE THEN STARTED TO STAND UP, TWISTED HIS ANKLE, BREAKING THE SMALL BONE IN HIS RIGHT LEG.
Injured was cutting a hole in a polydeck screen to remove the screen from the vibrator, while he was cutting the screen with a utility knife the knife slipped from the screen striking his right hand. Laceration 5 stitches.
Received a CWP claim on 8/8/2013. Employee last worked on 9/13/2004 and is no longer employed with the company. Accident date of 9/13/2004 is to accommodate online reporting per MSHA (Denver) instructions.
INJURED WAS WORKING ON TOP OF THE EXPLOSION DOORS OVER THE CYCLONES OF THE THERMAL DRYER. HE STEPPED BACKWARDS OR HIS FEET SLIPPED CAUSING HIM TO FALL SIX TO SEVEN FEET STRIKING HIS RIGHT ANKLE AND LEG ON A METAL BEAM.
INJURED ALLEGED HE HAD STRAINED HIS BACK WHILE INSTALLING PALE MOUNTED LIGHTS AT THE RIVER PREPPLANT.

INJURED WAS CHANGING HAMMERS IN A JEFFREY CRUSHER, AS HE WAS PICKING UP HAMMER TO HOOK TO THE ELECTRIC HOIST, THE HAMMER SLIPPED BACK MASHING HIS LEFT FINGER AGAINST THE CRUSHER.
INJURED HAD STARTED MOUNTING A DOZER FROM THE STEP ON THE PUSH ARM. HIS RIGHT FOOT SLIPPED OFF THE STEP CAUSING HIM TO STRIKE HIS RIGHT RIB CAGE ON THE PUSH ARM OF THE DOZER. INJURED STATED TH AT HE HAD A 3 POINT CONTACT WHEN HIS FOOT SLIPPED OFF THE STEP.

INJURED WAS CLOSING RAILROAD CAR DOORS USING A COME-A-LONG AND A BAR. THE CARS HAD SOME FROZEN MATERIALS IN THE CARS, AS HE WAS USING THE BAR TO HELP CLOSE THE DOORS, THE BAR SLIPPED OUT CAUS ING HIM TO FALL BACKWARDS PUTTING ALL HIW WEIGHT ON HIS LEFT LEG & ANKLE CAUSIN GHIM TO FALL TO THE GROUND.
INJURED WAS PUTTING A NEW SET OF BOLTS ON A VIBRATING SCREEN AND AS HE WAS TURNING THE SHEEVE WHEEL WITH A PIPE WRENCH, THE BELT WENT ON THE SHEEVE WHEEL. IT TURNED BACKWARDS AND CAUGHT HIS FI NGER ON THE MECHANISM CUTTING HIS FINGER.
INJ WAS UNPLUGGING SAMPLE CHUTE PIPE AT THE HEADEND OF THE HIGH 7BL SAMPLE BELT WITH A BAR WHEN HE FELT & HEARD A POP SOUND IN HIS RIGHT RIB CAGE
INJURED JUST CHECKED THE FREEZE TREATMENT TANKS AND WAS WALKING TO THE LOADOUT WHEN HE STEPPED IN A RUT THAT WAS COVERED AND FALLING TO THE GROUND. OUND.

INJURED WAS CLEANING UP IN THE LAB AS SHE BENT ODOWN TO CLEAN A DOOR HER LEFT KNEE POPPED AND SWELLED UP BEFORE THE END OF HER SHIFT. EE WENT TO DR. AND STARTED WORKING LIGHT DUTY 6-26-98.

EMPLOYEE WAS HELPING CARRY A JOINT OF 2" PIPE UP THE CATWALK ON THE REFUSE BELT. THE END OF THE PIPE HIT A HANDRAIL CAUSING THE EMPLOYEE TO STRAIN HIS NECK AND SHOULDER. THIS MAN DID NOT STAR T MISSING WORK UNTIL 4/17/90.
