EE WAS TRYING TO ADJUST BELT ON HELPER SIDE DUST SUCTION. HIS RT HAND SLIPPED OFF THE ADJUSTMENT AND WENT INTO THE RUNNING PULLY. IT BROKE HIS MIDDLE FINGER ON HIS RT HAND.
CLOSIGN AIR LOCK DOOR AT FAN SLIPPED OUT OF HAND AND CLOSED ON WAS FINGERS OF RIGHT HAND.

WHILE INSTALLING A ROOF BOLT HE ALLOWED HIS FINGER TO BE CAUGHT UNDER PRESSURIZED STEEL.
EE WAS REPAIRING A ROOF BOLTER WHICH HAD BEEN BLOCKED FOR REPAIRS. WHILE REPAIRING THE ROOF BOLTER HE LOOSENED A LOCK NUT AND THE ATRS SLIPPED OFF THE BLOCKING HITTING HIM ON THE LEFT SIDE OF THE BODY.
