The employee was in the process of preparing a wing wall on a feed hopper for rigging when the incident occurred. EE was attempting to free up a shackle that was stuck on the structure by pulling on the chain slings attached to it. The shackle sprung and hit the EE on the nose. EE was taken to local hospital where EE was evaluated, treated and released with no restrictions.

EE was welding the frame onto water tank where EE was on a ladder that was not folded out. EE was repositioning when EE lost footing and fell to the ground resulting in a laceration requiring stitches.

While working on Mack water truck IW put a flashlight in mouth to free up their hands. IW did not crouch down low enough and the flashlight pinned against the fender well prying upwards on the flashlight and downwards on EE's tooth. EE immediately felt a pop and piece of the tooth chipped off the back side of the lower left front tooth.

THE EE WAS HELPING THE MECHANIC FROM JW BURRESS TO REPLACE A POTERY HEAD MOTOR ON A DRILL TEC DRILL. EE WAS LIFTING THE MOTOR FROM THE ROTERY PLATFORM TO THE SERVICE TRUCK. THE MOTOR WAS AOBUT 38" ABOVE GROUND LEVEL WHEN EE PICKED THE MOTORUP HE TURN ABOUT 180 DEGREE AND SAT THE MOTOR DOWN.
