EE WAS REPLACING A GREASE FITTING BENT DOWN FROM CATWALK STEPED TO BEAM SLIPPED FELL ACROSS H BEAM AT CHEST KNOCKING WIND OUT OF HIM SAID HE WAS OKAY WEEKEND THE REST OF DAY THAT NIGHT HE WAS HURTING AND WENT TO HOSPTIAL AT THIS TIME HE IS HOSPITALIZED.

EMPL CLIMBED UP ON WORK PLATFORM WITHOUT SAFETY BELT AGAINST HE SUPERVISORS INSTRUCTIONS. HE LOST BALANCE WHILE ON CONCRETE WALL SIDE OF HOPPER & FELL INTO HOPPER HITTING HEAD AGAINST CONCRETE WALL OF HOPPER AS HE FELL. AMBULANCE TOOK HIM TO HOSPITAL WHERE HE RECEIVED 12 STITCHES & WAS RELEASED BY 1 PM.
